I asked for a return call in regards to the NOV issued, on this well, for non-production and would like to know their plan of action. She states that either Dick Salsberger, Jordan, or Gary would call me back.

Mr. Jones states they will have rods and tubing steamed by start of bad weather. He is thinking within the next month this should be completed. Further stating that the well had paraffined up.

1060636975 2009-12-29 Administrative Inspection No Violations Spoke with Mr. Dick Salsberger, G-R Contracting, by telephone at the office today at 11:05 AM, in regards to well status. He states that a rig will be placed on the well within the next month. They had a rig working around area 3 months ago, but found that the Gordon Rig would be needed for job. The pumper stated the well is jammed with paraffin and had never been treated when they bought the well.
1202139173 2009-12-10 Production Wells No Violations A follow-up inspection conducted with Mr. Robert Stonerock, South Region Oil & Gas Supervisor revealed non-compliance. The well is still idle. The engine has now been removed from the pumping unit. Note: light snow cover observed at time of inspection. At the conclusion of the inspection, the landowner voiced concerns regarding the well status.
1202210712 2010-11-10 Production Wells Violations Noted The Division received a call from the landowner, in regards to land rights issues (no statutory authority) and alleging that an employee working on the well stated we hope that the state inspector doesnt see this. An inspection revealed service rig has been placed on well. Rods and tubing were pulled from well bore and have been steamed in a pit with no liner. Pollution and contamination measured 20 x 20. Notified Mr. Jordan Jones by telephone at 3:10 pm. He was instructed to remove P&C from the site and burial is not permitted. Mr. Jones agreed to the terms and states they should have used a liner because they always do.
1238699693 2009-03-10 Production Wells Violations Noted An inspection revealed that the well is idle. The well does contain production equipment to include pumping unit with an engine. Black plastic flowline observed routed from the tubing side of the well to a 100 bbl tank. The tank appears to be empty and the polish rod shows signs of non-production. Thick brush and vegetation has grown around the well and the pumping unit. A valve was observed closed on the casing side with no attached flowline. At the time of the inspection, RBDMS shows Historic Owner with no reported production. Identification placed at the well shows that G-R Contracting is the well owner. A GPS reading was obtained for the well and the tank battery. The location of this well has been verified with Columbus. Plug or produce this well. Your attention is also directed to ORC 1509.11. Our records do not show a statement of production of oil, gas and brine for this well. Please submit production records to the Division of Mineral Resources Management.

The options provided were to: plug, produce or apply for temporary inactive status (brief explanation). Mentioned to him that I would return from vacation in two weeks and will call once I return.

1596688029 2009-09-02 Administrative Inspection No Violations At 10:25, I called G-R Contracting Inc at their office number listed in RBDMS. I spoke with Dick about their intentions with this well. He was advised of non-production and confirmed they own the Snyder #6. Also mentioned to him that permit number listed on their ID is not correct and has been resolved by Columbus. He states that this well was part of a 40 well package to include wells in Athens and Washington counties. They have had interested parties, but would talk with Production Supervisor, Jordan.

1672510952 2010-11-19 Plug / Plug Back No Violations A follow-up inspection revealed compliance. No observed P&C and rods and tubing have been placed back in well. The service rig remains at the location. Made an attempt to meet with the landowner to discuss inspection results. The landowner was not available therefore, a business card was left. The 100 bbl tank appears to have been replaced a used 100 bbl tank and newly constructed earthen dike. 1.25 black plastic routed from the well to the tank. Note: Well status remains inaccurate and shows PA in RBDMS.
