0072109613 2011-10-26 Production Wells Violations Noted An inspection conducted with Dan Goins, Inspector 3 for the South region Division of Oil and Gas, presented several infractions. The dike around the tank battery is not capable of holding fluid, there two separate tanks that have no dike, there is no identification on the tank battery, the identification on the well head is not legible, there is no dike around the well head, gas is being vented from the well head and production for the year 2010 has not been submitted. The dike surrounding the tank battery needs to be re-built. Dikes need to be built around the two separate tanks on the site or tanks need to be removed. A dike also needs built around the well head. Identification needs to be posted on tank battery and well head compliant with 1501:9-9-05 A(10). A flare needs to be built for the gas that is being vented compliant with 1501:9-9-05 B. Production records for 2010 need to be submitted.
0621054755 2012-06-27 Production Wells No Violations Dikes have been built around tanks and around well head. There are two flow lines coming in to the tank battery as it is shared with the other well on the property (p#62336). Contaminated material has been removed from around well head. Both large and small tanks that were previously on the premises had been removed. A flare has been constructed and this well is connected to it. Electric service boxes have been replaced with new ones. Production records were not submitted for the year 2011. Spoke with Monica Schmelzer on 6/28/12 via telephone and there is still a discrepancy about the identity of the well between Jeanie Enterprise and the States records. She then quoted to me the production for the well.

He alleges that Jeanie has no records of oil sold from the lease. There is no production reported for the year 2010. This was addressed in the previous compliance notice inspection # -72109613.
1323705173 2011-12-12 Administrative Inspection No Violations Received voicemail from Mark Schmelzer on Friday 12/8/11 stating that he had been locked out of the property in which this well, and the Ford HN #4 (permit #62336) are located. On Monday 12/12/11 I went to the site, and there was a lock on the gate. I called Chris Eiben at 8:21 am to find out why. Mr. Eiben stated that he had tried to discuss the royalty issues and the electrical issues with the company to no avail and thats why he locked them out. He had also stated that the State Fire Marshal, and a representative from South Central Power inspected the electric situation and had condemned and disconnected it. Spoke with Mark Schmelzer at 1:31 pm, and he stated that right now his lawyer and the property owners lawyer are handling the issues.

All 3 of these wells are now plugged. This satisfies the compliance notice.
