0107120587 2017-07-14 Orphan No Violations Orphan well status inspection: Upon inspection, it is observed that the well is leaking gas at a greater rate than previous inspections. Gas is leaking through the fluid column inside the 6.25 diameter casing. Gas is also migrating outside of the wellbore and bubbling out of the streambed approximately 3 downstream of the well casing. No brine or oil was observed leaking from the wellbore this day.
0374946278 2016-06-28 Orphan No Violations On location to check the status of this orphan well. Upon inspection, it was observed that the well is not currently leaking. There is woody debris piled up around the well from springtime high water events. The casing appears to be canted slightly due to stream bank movement and erosion over the years. This well and P#62600 have been approved for U.S. Coast Guard funding to plug. New photos were taken of the well location and ingress/egress.
0448261774 2018-05-01 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Emergency Response Staff member Nate Lane. The purpose of this inspection is to determine if recent flooding events in the area adversely affected this well. Upon inspection, it is observed that the conditions at the well location are similar to pre-flooding conditions.
0686183749 2015-09-29 Orphan No Violations My inspection today was with the Federal EPA Inspector, Joe Fredle. I showed him the well in the stream that was leaking slight amounts of oil and bubbling gas and he agreed it would qualify for Coast Guard funds.
1025710696 2018-02-27 Orphan No Violations Inspector on location to attempt to gauge the depth of this orphan well for development of a plugging plan. Upon attempting to run the weighted line into the well, it is observed that there is an obsruction present in the wellbore at a depth of 20. Gas is flowing steadily from the 6.63 diameter casing.

1126198606 2015-07-21 Orphan No Violations My inspection this day was conducted with DOGRM Inspector for Hocking County, Kip Thompson. We found this well in Queer Creek about 6 out from the bank, in water about 3 deep. There appears to be 8 5/8 OD casing with 6 - 7 diameter casing inside the larger casing. The well is leaking gas from the creek floor in a circle around the well and there is an oil sheen present. The top of the casing is 5 above water level and open to the atmosphere. Rick Queen, who works for Ohio EPA, is the adjoining landowner to the northwest. He explained that Queer Creek is one of the few streams designated by law in Ohio, as both a Cold Water Habitat Stream/Exceptional Warm Water Habitat Stream and says there are many species of fish, insects, and amphibians that are only found in this kind of stream. OEPA has been monitoring the stream for many years and have documented, although not measured, the elevated brine levels and presence of hydrocarbons. There is no record of this well in our database but the old Ohio Fuel Gas map shows it as an oil well, drilled to 560 in the Berea Ss. It shows several more wells in the area and Mr. Queen says he believes they were originally drilled north of the stream but that the stream has moved considerably since these were drilled. The access road to #2620, which was plugged in 2012 and was located 250 north of #2599, is still well defined and can be used. Kilbarger acquired this property in 2010 after trading this property for the plugging of #2620. It should be noted that Rick Queen is not the owner of the property, as reported earlier. Pictures taken, SEM is 206.
1238872126 2018-02-16 Orphan No Violations Visiting this orphan well location with OWP Manager Gene Chini and ODNR-Legislative Representatives. The purpose of this site visit is to familiarize ODNR-Legislative with some of the more challenging projects that the OWP is involved with. Due to high streamflows this day, the well can only be observed from the stream bank.

1511202536 2017-11-17 Orphan No Violations Orphan well field work, accompanied by DOGRM staff Paul Carder, Rob Lowe, Luke Smith, and Josh Schiering. The purpose of this site visit is to evaluate the orphan well location surveying, radiation safety, and environmental considerations. This well is part of the Hocking #1 plugging project which is scheduled for completion in 2018.
1526394411 2018-02-13 Orphan No Violations Orphan well status inspection: Inspection this day finds well to be in similar condition to previous inspections. The well is steadily leaking gas to the atmosphere out of the 6.63 diameter casing. The PhD6 4-gas meter detected an ambient reading of 0.10 ppm CH4 and an LEL of 7 at a distance of 3 from the well. Recent flooding does not appear to have affected the well casings.

1998670115 2014-11-10 Orphan No Violations I found a well casing wile doing a orphan well investigation. The well consists of a 8 casing that has a plate welded on top. There is a swage attached to the plate. The well is approx. 100 from the well with API number 3407362600. This well does not appear to be leaking. I have filled out a site matrix for this well and shared the information with program manager Gene Chini.
