0410708434 2012-09-18 Production Wells No Violations Inspected the three shut-in ARTEX wells (Trepanier 1&2, API 34079201020000 & API 34079201380000) and Garrett #2 (API 3407920206) with Don Huck (Exploration Manager for Artex) and Dean Harris (oiler for Artex). Found all three wells were leaking gas between the 2-3/8 inch production tubing and the $-1/2 inch intermediate csg at the tubing ring on the casing head. I expect them to contact me by this coming Thursday 09/20/2012 on what they plan to do or that they have corrected the problem. Don contacted me on Wednesday 09/19/2012 that the leak has been fixed. I field checked the wells with the Honeywell PHD6/PID gas meter and doused with bottle water covering where the leaks occurred and there was no reading on the meter and no bubbles where the leaks occurred earlier.

1347239323 2012-09-06 Production Wells No Violations Traveled to the area in which this well was located according to GPS. Spent an hour ground checking to locate the well. Located a well head and storage tank from a distance on an adjacent hill. Couldnt find an open access without climbing over electrified fences, and locked and chained gates. I will call Artex tomorrow (09/07/2012) to gain access to this area.
1347912829 2012-09-11 Production Wells No Violations Don Huck left me a message about our meeting this Thursday at 0930hrs on the status of this well (producing from the Beekmantown Dolomite), API 34-079-20138 (Trepanier #2 ) and API 34-079-20206 (Russell Garrett #2). The latter two are producing from the Ohio Shale. These three wells have been connected to a pipe line to originally the Goodyear plant in Jackson. Theyve been shut in since 2007 when no production has been reported to date (2011).
1348151782 2012-09-20 Production Wells No Violations Inspected the three wells (Trepanier #1 &2 (PN 20138), and the Garrett #2 (PN 20206) that were found to be leaking on my first trip to this site (09/18/2012. Poured bottled water and used Honeywell PhD6 gas meter to monitor for any CH4 gas emissions from where the original leaks occurred plus examined and scanned with the gas meter for any emissions around the surface tubing for each well. I found no bubblng of gas from the fittings or emissions at this time and the three wells passed inspection.
1351092026 2012-10-23 Administrative Inspection No Violations I tried to contact Don Huck, Manager for Artex Oil, by phone to gain access and perform inspection audit for this well, PN 20138, Trepanier #2, and PN 20206, Garrett #2, for Artexs permit application to change the status of these wells from producing to Temporarily Inactive. I left a voice message. Ill try again tomorrow.
1353787717 2012-11-08 Administrative Inspection No Violations Performed Temporary Inactive (TI) Audit for Artexs TI application. Dean Price, Artexs well tender for this well, accompanied me on this audit. Checked all valves to be sure they were off; no leaks were measured with the Honeywell PhD 6 gas meter nor observed by myself; all gas lines on the sales side of the well head including the gas line to Jackson were empty according to Dean Harris and have been since this well was shut in. The only glitch in this audit is this well is providing domestic gas to lease owner from the back side of the casing. Besides this issue the well meets the requirements for TI status.
