0243621740 2015-04-07 Production Wells Violations Noted Asked to check on status of the well by Cynthia Marshall. Well equipped with 4 1/2 production casing and a swage reduced to a 2 line, well also equipped with two stepdown regulators. Valves on the well were closed and the 1 1/2 plastic line was disconnected at the well head. No production has ever been reported for this well and it is not capable of production. Well shall be plugged.

The compliance notice was issued on 4.7.15. There has been no attempt to bring the well into compliance. The contact number for the owner of this well is a non working number.
1013602137 2013-08-27 Field Inspected, Well Not Found No Violations I tried to find and inspect this well to determine if there was any possibility that it was contributing to the surface water contamination of the Buckeye Creek where it crosses State Route 776 just 100 feet south of the intersection of State Route 32 and State Route 776. I could not find the well. This inspection was a result of a possible oil spill reported on 8/26/2013 to Dave Claus, who contacted me to go and inspect the incident. It was not an oil spill but I wanted to be sure none of the wells in this drainage area did not contribute to this incident.
1149795999 2018-10-11 Production Wells No Violations My inspection revealed the well consists of a 4.5 inch diameter wellhead with a 4.5 inch diameter to 2 inch diameter swage. On top of the swage was a 2 inch diameter valve that was in the off position. This well is not hooked up to any production line. There is no phone number listed in RBDMS so I could not contact the owner of the well regarding the violation.

1381494591 2013-10-10 Administrative Inspection No Violations A Mr. Dave Liscaney contacted me about this well on his newly acquired property in Jackson County, Scioto Township. I checked RBDMS for current ownership and informed him that even though he is the owner of this property, that the ownership of the well did not transfer to him. According to him he tract property ownership of the property from Mr. Hatten to present. He purchased the property from Scioto Land Company. I informed him that he would need to get the courts or whoever to vacate the current ownership in order for the well to be transferred to him. Further he should also determine if there are any liens or other financial responsibilities associated with the well. Then I asked him if there was a pipeline from the well and if so where did it go? These are all things he did not consider plus the financial responsibility of now being the designated owner. I had him contact Erica Freeman or Vickie McKinney for other ownerships issues related to this situation. Ill inspect the site tomorrow.
1381925376 2013-10-11 Production Wells No Violations New owner, Mr. Dave Liscaney, indicated to me yesterday that he was having a excavation contractor grade an access road from John Hoops road to the wellhead so maintenance work could be performed. This was an opportunity to locate this well and inspect its condition. It has the appropriate identification. However Mr. Hatten may be deceased and his farmhouse that this well presumably supplied gas is abandoned for the moment. Hardware on the gas wellhead needs some tender loving care and Im not sure if it is capable of providing domestic gas supply. The wellhead at the surface starts with 5-3/8 inch casing rising above the ground into a collar. Above the collar is a 5-3/4 inch to 2-3/8 inch swedge. On top of this fitting is a ball valve (open position) and then rising above this valve a 2-3/8 inch tubing rises to a t-fitting. The top of the fitting has a plug in it. On the side fitting there is another 2-3/8 inch tubing to a 2-5/8 inch to 1-3/4 inch swedge fitting. From the small end of this fitting is a 1-5/16 inch tubing attached to a small vertical knock out cylinder. This cylinder has a plug at the bottom. Near the top side is another 1-5/16 inch port with a similar sized tubing attached and ending in a t-fitting with a low pressure regulator mounted on the top. Also from the t-fitting is another ball valve, then a pressure gauge finally ending in an orange colored plastic tubing which plunges into o the ground.
1622795300 2015-10-06 Production Wells No Violations Follow up to #243621740. Identification at well, but lines are not connected. Cynthia Marshal in bonding and well ownership had asked me to inspect this well. I spoke with Cynthia 10/9/15 and she informed me the well was in Chiefs Order for ownership issues so I could cancel my idle violation.
