0125100270 2008-01-12 Drill / Deepen / Reopen No Violations Surface cement job. Spud date 1-11-2008. Ran 65 of conductor in a 11 hole. TD 1144 set 1095 of 8 5/8 plus 11 landing joint. I got called out at 8:30pm on the 12th and cement trucks were late. Cement trucks arrived at 3am and got stuck at location for an hour. Had good circulation and good cement returns to the surface. I left the location at 6:30 am on the 13th.

1201614705 2008-01-28 Drill / Deepen / Reopen Violations Noted Pit liner on the mud pit has fallen in on one side. Oil and other drilling fluids are in contact with the soil. Oil in puddles around the well head. Wellhead is leaking some gas. I tried to call the well owner but could only get an answering machine. I left a message but was cut off due to machine running out of memory space.

I called Stan Holler and he informed me that work has been arranged and that the weather is making it difficult to get a water truck in to the location. I suggested dragging a frac tank with a dozer to the location and pumping the pits into it until it dries up.

Well has 2 tubing ran. Well is not connected to any production equipment or production lines. Vegetative cover has not been established. 95% bare ground. Site is very rough with ruts and mud over entire location. Pit liner pieces are strewn around location. Sediment is moving off location and into the wood line. A half joint of 8 5/8 casing is laying in mud along access road. Access road does not have any diversions or means to control erosion. Road is a low cut through the woods. This traps the surface water and allows it to form a channel with in the road. Place Identification at wellhead. Remove all debris from well site. Grade well site. Seed and apply mulch to establish vegetative growth on location. Follow Best Management Practices For Well Site Construction. Follow Permit Restoration Plan.
1702146215 2008-01-11 Drill / Deepen / Reopen No Violations No permit on location. I told them they need the permit on location. Rig has not spuded yet. Pit is positioned to allow for 60 of flow line. They are drilling with air. I talked to the driller about a min. of 60 of conductor. I advised them to take the conductor down until they hit something solid at least 60. I gave the rig crew a copy of Ohio Oil& Gas Laws & Rules.

1800969489 2012-10-01 Final Restoration No Violations Well is shut-in. Restoration of site is slowly recovering. Owner claims he has tried reseeding several times. It appears from my visit that the pH of the soil may not be conducive to support plant growth. May need to add some amendments such as lime to facilitate plant growth. pH could be a problem since rainwater drains over some exposed coal material above this site.
1817043872 2009-12-14 Preliminary Restoration No Violations Seed and mulch have been reapplied to location. 85 % of location has grass growing. Erosion ditches have been filled in. Chucks of fertilizer have been applied to location. No identification at well. No production equipment or production lines are at well site. Well is equipped with tubing and a valve with a gauge. Gauge reads 120psi. A small gas leak at the 8 5/8 collar and the 8 5/8 by 4 1/2 swedge. No explosive levels were detected with the gas detector. I called Stan Holler (well owner) and left him a message regarding the leak and for him to call me with a time frame on getting the well in production.
1826864904 2009-10-08 Preliminary Restoration No Violations New Grass is growing. Site still has some bare areas with no vegetative cover. Need to reseed bare areas with a variety of seed types to allow better vegetative cover. Well head has slight audible leak. No explosive levels detected with gas meter. Still need to grade erosion site and seed and mulch. No identification, No production equipment or production lines. Called and talked to Stan Holler Well owner and we discussed the work that needs to be done. Stan asked if he could install the id when he runs the production lines. He is bidding out the line work now. I agreed as long as the work gets accomplished soon.
1874207679 2010-08-24 Preliminary Restoration No Violations Vegetation established. Trees starting to grow on location. No substantial erosion. Inspected site with Robert Stonerock South Region Supervisor. Well is connected to production line. No identification on wellhead. Stan Holler ( well owner) was on the lease during inspection. Robert and I talked to him in detail about corrective actions that need to be taken. We also discussed the tank battery and the dike drain which does not have a valve or other means to close the drain. No identification on the tank.

2120889198 2008-01-31 Administrative Inspection No Violations Talked to Stan (Owner) He said that a contractor is meeting at the site on 2-1-08 to look at it and give him a price. Stan said that he will call me next week to give me an update. I explained what I wanted right now was the fluid in the pit with the bad liner to be pumped out and hauled to a disposal well. I told him the restoration time lines on his project 5 months to pump and reclaim the pits and 9 months to reclaim the location.
