0215630671 2016-02-17 Exempt Well Pre Transfer No Violations Pre exempt transfer inspection for domestic use well requested by Columbus. Upon arrival at the wells site I found a gas well that had no flow line connected to any of the casing heads. The seven inch casing head had one 2 fitting with a cap in it to shut it in. The other fitting in that casing head had a nipple with a valve that was in the open position. The 4 inch casing head had both fittings closed off by use of valves. The tubing had a valve, then a tee with one side plugged and the other side reduced down for a pressure gauge. There were no production lines coming off the well, even just a line for house gas. The well appeared to be in an idle state. The only ID at the well was a blank Knox energy sign. There was also about 18 joints of 2 laying on the ground by the well.
0372030344 2008-04-24 Drill / Deepen / Reopen No Violations Spud date 4-24-08. Equitable Drilling is overseeing the project. Highland Rig #5 is drilling the well. Rig is a Speed Star 185. BJ is cementing. Weatherford is Logging the hole. 120 9 5/8 conductor in a 12 3/8 hole. TD 1255 8 5/8 hole. 1220 7. Shoe at 1220. Baskets set at 120 & 240. Fire extinguisher at the entrance to the dog house. Had circulation and cement to surface. Cement fell back and was grouted from the surface. I was called out at 10pm on 4-24-08 for the surface job. It did not start until 4am due to logging problems and casing problems.
0919235668 2009-10-06 Preliminary Restoration No Violations Good id. Site is graded with good vegetative cover growing. A piece of plastic liner 15 by 5 that was lining the ditch from the well to the pit is still in place. I have called the company and they are working on removing the plastic liner. 18 joints of tubing are on the ground near the well. Well is equipped with tubing. No production equipment or lines are on location.

Property ownership change is in the process. New owner contacted me on 09.21.2012 about what he needed to transfer ownership from Knox to his company. Gave him information and had him contact Erica Freeman in Columbus about getting the latest forms for changing ownership of a well.
1403205604 2010-03-01 Administrative Inspection No Violations Called and talked to Terry Wade at 11:40am Re: Well needs identification. I also requested a written time line on getting the well in production or plugged. Terry said that they have not had a chance since taking over the wells to evaluate and see if it is capable or needs plugged. Terry told me that Bill Mitton is on vacation and will be back in a week. He will have Bill get in touch with me.

1548938410 2009-02-17 Preliminary Restoration No Violations No identification at wellhead. 2 inch tubing has been ran into the well. 18 joints of 2 inch tubing are stacked 5 feet from the wellhead. Pressure gauge on the 4.5 inch annulus reads 580 psi. The valve on the 2 inch tubing is closed. Both 2 inch valves on the 4.5 inch annulus are closed. Both 2 inch valves on the surface casing annulus are open. Pit has been closed. Grading at pit location is low and standing water is pooling in an area 75 feet by 30 feet by 8 inches deep. Pictures were taken. No seed or mulch has been applied to pit site. Well site is gravel.
