0763055173 2012-09-13 Plug / Plug Back No Violations Appalachian Well Surveys ran a bond log on the 4.5 casing. Packer inside 7 casing at 1095. Used casing cutter inside 4.5 casing at 1721 Measured depth. BJ Hughes Packer required 74,000 pounds to release it. While pulling 4.5 casing Brine flowed into lined pit. Packer was swabbing the well as casing was being pulled to surface. Wire line set a mechanical bridge plug in 6.25 wellbore at 1700 feet.
0923596264 2012-09-13 Plug / Plug Back No Violations Appalachian Well Survey on location and rigging up to run GR-CLL-VDL. Went to 2000 an logged hole. Survey found packer on 4-1/2 inch csg deployed in the 7-inch surface csg @ 1095. Decision was made to run casing cutter in hole and shoot the 4-1/2 csg at 1721 MTD. Cut csg. Slowly applied 75,000 lbs pull on 4-1/2 csg until packer at 1095 released. Started to bring 4-1/2 csg out after two joints foamy fluid started coming out between the 7-inch and 4-1/2 csgs. After removing 26 joints well unloaded gas and water blowing to the crown for a few minutes and then died. Got all 4-1/2 csg out from 1721. Packer at 1095 was Baker-Hughes. $-1/2 csg was in good shape. Went back to 1700 and set a bridge plug as a footing to the plugs this well. Shut down for night.

Conditioned open hole with gel and Lost Circulation Material (Flake and Cotton seed hulls) Set bottom plug in two stages from 1690 Pulled 10 joints tubing (average 33 each) Plug top 1090. Waited on cement 35 minutes no fluid loss at the surface inside casing. Pulled tubing to 110 and circulated cement to surface. Cement stayed at the surface.
1142645850 2012-09-12 Plug / Plug Back No Violations Arrived at the location. Berry Well Services service rig was set up and pulling the 1-1/2 inch production tubing. Pulled 61 joints including the approximately 20 mud anchor. Based on an average length there was about approximately 2000 feet. Went back down hole with a sand bailer attached to cable sounding for a port. Pulling this string back to surface didnt find anything from 1350 feet below ground surface to the surface. Went back into the well and went deeper to 1500 feet with the same assembly. Still found nothing. Placed cap on csg and called it a day.
1144367559 2012-09-14 Plug / Plug Back No Violations Started setting 1-1/2 inch tubing and went to 1690 to start conditioning the hole. Formation Cementing arrived on location and set up for plugging. Jon laid out a plan to plug the well, Everyone agreed. Started by using gel and LCM Cellophane (Flake) to condition the bottom of the hole above the plug set last night. This hopefully will help to seal off openings in the Ohio Shale and let the first cement plug stay without losing it in the formation. It went like clock work and finished the job around 1200 hrs topping cement close to the surface. It looks like surface cement was holding. Jon stayed to get Formations plugging report. I left.
1213641230 2008-06-09 Completion Testing Violations Noted Well has been blown back with out piping to the pit. This resulted in a high level brine being sprayed on the surface around the wellhead. The brine was off the chart for the high end Quantabs. The brine was bright orange in color. A 15 by 10 by 4 deep puddle of brine was collected near the pit. Wellhead is covered with gravel and the brine can be seen running out from under the gravel. The brine flow heads toward the puddle mentioned above. I informed the men overseeing the well that this must be cleaned up. Drill pit liner has fallen in on two places. Liner needs to be fixed or replaced.

I was un able to witness the cement job due to a Pipe Line Safety Training in Athens.
