According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owners name, county, township, well name and number, permit number for the well, and emergency contact number. I monitored the site around the wellhead and measured low concentrations of methane gas, less than 1.0 meter reading units. At the location of the gas leak, the detectors for methane gas and the LEL exceeded their maximum meter reading unit capabilities. According to 1501:9-9-05(A)1/1509:20 Well is insufficiently equipped to prevent escape of oil and gas. The owner shall repair or replace faulty equipment on the well to prevent the release of any contaminants into the environment. There is no tank battery associated with this well. Location of the wellhead is within 20 feet of the access road, Richland Furnace Road.

This well shall be transferred to current owner/operator. Assignment should be noted in deed when property transfer occurred between the sister and their parents, and ultimately her sister and Harriet McGlinchey nee Leasure.

1544955205 2008-08-15 Administrative Inspection No Violations Marvin Allmon 1562 Bert Harold Rd Jackson, Ohio 614-875-6875 called the Jackson Office to report oil in his pond. The pond GPS location is N38.96519 / W082.68114. The complaint was inspected on 8-15-08 at 11am. The pond had a black color with slight sheen to it. No crude oil smell was observed at the time of inspection. A sample of the pond water was collected and then evaluated. The black substance was not crude oil. The substance did not have an odor and did not stick together like oil does. The substance appears to be a form of algae. I was unable to link any of the complaint to oil and gas well activity. I referred the Allmons to the Jackson County Soil and Water Office for information on their pond.
