0047393203 | 2018-05-15 | Production Wells | No Violations | On the day of my inspection I found this well equipped with a walking beam pumping unit. Well is equipped with 4.5 inch diameter casing, 1.9 inch diameter tubing, and rods. Gas can be heard escaping from the polishing rod. Jim Posey was contacted and he was onsite within the hour to investigate and repair. I had him suck water out of containment. Production equipment includes poly tank and horizontal seperator. Water inside containment tested 850 ppm chlorides. |
0168797763 | 2017-08-28 | Production Wells | No Violations | Today I am onsite to test the bottom of the excavated area outside of the production berm. Jim Posey with Samurai Corporation removed the top 8 inches of soil that had been contaminated with brine. I placed soil in a clean container then added some deionized water to create a slurry. After 5 minutes I placed a Quantab into the solution. The water tested for 56 parts per million chlorides. Jim plans to backfill the area with clean dirt. He is still working on a solution for the area inside the berm. |
0534833306 | 2016-09-14 | Production Wells | No Violations | On the date of my inspection I found a tank battery to include two properly identified 100 barrel tanks, a horizontal separator, a drip, and a production meter box with a current chart within an earthen dike. The well is equipped with a pumping unit, rods, 1.5 inch tubing and 4.5 inch casing. The tubing and casing valves are in the open position. The production records are only current through 2014. |

0846015902 | 2017-10-12 | Production Wells | No Violations | Today I am onsite to follow up on clean-up efforts related to a release that occurred on 8/22/17. Samurai has placed new gravel within the dike and the newly seeded areas are going healthy grass. A 38.75 barrel poly brine tank has been placed within the dike but has not been plumbed into the system yet. Both 100 barrel tanks have been hauled off site. Clean up efforts for this location are now complete. |
0886267916 | 2013-01-31 | Production Wells | No Violations | The tank battery containing two 100 barrel production tanks and a horizontal separator has had a new SPCC dike built around it. There are no violations at this time. |
1244639629 | 2013-05-16 | Administrative Inspection | No Violations | I was in contact will the pumper for Samurai regarding the 2 compliance notices that were issued for some issues at their wells. This well had a brine filled hole that has currently been pumped out but has not been filled currently but does have a fence surrounding it. A contractor will be onsite today to see how to fill the holes. Signage for the wells has also been ordered and will be installed next week. I will conduct a status check to determine if the wells are brought into compliance. |
1312037076 | 2002-10-31 | Production Wells | No Violations | Routine inspection revealed no violations at this time . |
1354829241 | 2012-12-06 | Production Wells | No Violations | Inspection of the tank battery contained two 100 barrel production tanks and a horizontal separator. Note: the top of the buried cement brine storage tank is broke along the east edge . The tank is not being used at this time . It appears that the tank has not been used for quite sometime . The well has a pumpjack on it and is located in the backyard of several houses away from the tank battery. The well is surrounded by a fence. There are no violations at this time. |
1366915472 | 2013-04-24 | Production Wells | Violations Noted | I was conducting a status check of this well. The pumpjack well is fenced in behind several houses. The tank battery is located on an adjacent road and contains 2 production tanks and a horizontal separator. The well is contained within a dike but there is a hole filled with brine and oil where a former brine vault was placed. The well is also lacking legible signage. The hole needs pumped out and filled in to limit the chance of someone falling in and the well needs proper signage. |
1369840952 | 2013-05-28 | Production Wells | No Violations | I was on location to check the status of some work done to fulfill the requirements of a compliance notice issued for this this well. I was in contact with the Samurai well tender and he stated that the well has been brought into compliance. A new dike surrounding the tank battery has been installed as well as new properly labeled signage. The hole in the tank battery filled with brine has been filled can covered. The well is in full compliance and there are no violations at this time. The compliance notice can be closed out. |
1395083434 | 2017-06-05 | Production Wells | No Violations | On the date of my inspection I found the production equipment to include two properly identified 100 barrel tanks, a horizontal separator, a drip, and a production meter box with an orifice plate and a current chart within an earthen dike. The well is equipped with a pumping unit, rods, 1.5 inch tubing and 4.5 inch casing. A 2.375 inch production line links the tubing and casing. The tubing and casing valves are in the open position. I found gas actively leaking from a 90 degree elbow in the production line coming from the wellhead into the separator. I called Jim Posey with Samurai Corporation who came out to assess the leak while I was on site. He indicated that both compressors for the wells in the area stopped working today so all of the lines have higher than normal pressure. He said that he would fix the gas leak as soon as he could relieve the pressure on the lines. The production records are current. |

1504618909 | 2017-09-05 | Production Wells | No Violations | Today I am following up on cleanup efforts following a spill from the production tanks for this well. Samurai Corp has steamed the two 100 barrel tanks and removed them from the diked area. They have backfilled the excavated area outside the dike and re-seeded the area. They have started to scrape the surface of the diked area to remove the oil saturated soil and will be taking the soil to a landfill. |
1505759393 | 2017-09-18 | Production Wells | No Violations | Today I am onsite to follow up on clean-up efforts related to a release that occurred on 8/22/17. Samurai has scraped up more of the contaminated soil from within the diked area into a pile. Jim said that he would remove the soil pile this week and take it to a disposal facility. He also said that the two 100 barrel tanks that were steamed and placed nearby would be gone this week as well. Grass has already begun to grow on the area outside that dike that was excavated, backfilled with clean fill, and seeded. |
1506343609 | 2017-09-25 | Production Wells | No Violations | Today I am onsite to check on the progress of cleanup efforts following a spill. Samurai has removed the contaminated soil from within the diked area and has replaced it with lime and new gravel. The two 100 barrel tanks are still onsite waiting to be hauled off. The area outside that dike that was reseeded is now covered with healthy grass. The flowline from the horizontal separator to where the tanks used to be is leaking gas. I left a message for Jim Posey regarding the leak and advised him to either shut in the separator or plug the line. |
1695167835 | 2016-12-09 | Production Wells | No Violations | On the date of my inspection I found a tank battery to include two properly identified 100 barrel tanks, a horizontal separator, a drip, and a production meter box with an orifice plate and a current chart within an earthen dike. The well is equipped with a pumping unit, rods, 1.5 inch tubing and 4.5 inch casing. A 2.375 inch production line links the tubing and casing. The tubing and casing valves are in the open position. The production records are only current through 2014. |
1801377072 | 2006-02-15 | Production Wells | No Violations | Inspection of the tank battery revealed no violations . Didnt inspect the wellhead at this time . Note: the top of the buried cement brine storage tank is broke along the east edge . The tank is not being used at this time . It appears that the tank has not been used for quite sometime . No violations at this time . |
1887662915 | 2014-05-05 | Production Wells | No Violations | I was on location with Inspector Dylan Pendleton to perform a status check of this well. During my inspection I found the tank battery to contain two 100 barrel production tanks and a horizontal separator has had a new dike built around it. A condensate flow line for the well has been dug up near the road and is exposed in the drainage ditch. The landowner performed some landscaping and dug up the ditch and the line. Jim, the well pumper, was on location and I instructed him to rebury the line so that it is no longer sitting exposed in the ditch within the week. |
