0336713056 2017-04-06 Production Wells No Violations On the date of my inspection I found the tank battery to include one properly identified 210 barrel tank, one 100 barrel tank, a vertical separator, two drips, a production meter box with an orifice plate and a current chart, and a residential diaphragm meter, within an earthen dike. The 100 barrel tank has a plug in the heater tube. The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing and 4.5 inch casing. A 2.375 inch production line ties the casing and tubing together. Both casing and tubing valves are in the open. A pressure gauge on the production line reads 70 pounds per square inch. Production records are up to date. The 30 barrel poly brine tank that was removed from the tank battery during remediation work this summer is still onsite near the tank battery. I have still not received the truck manifests from the contaminated soil removal operation during the summer of 2016. I have spoken with the company and requested these documents.

An area with approximate dimensions of 7 feet deep by 4 feet wide by 4 feet long was removed. This area is now okay to backfill and the berm can be reconstructed.

The 100 barrel tank has a plug in the heater tube. The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing and 4.5 inch casing. A 2.375 inch production line ties the casing and tubing together. Both casing and tubing valves are in the open. A pressure gauge on the tubing line reads 40 pounds per square inch and a gauge on the casing reads 70 pounds per square inch. Production records are up to date. There is some peatsorb on the ground around the wellhead to clean up a recent leak. There are some scrap flowline sections and a tank ladder and walkway on the ground near the production equipment. Jim Posey with Samurai is working on getting rid of the scrap.
0452924675 2016-08-29 Production Wells No Violations On the date of my inspection I found the tank battery to include one properly identified 210 barrel tank, one 100 barrel tank, 30 barrel brine tank, vertical separator, two drips, a production meter box with a current chart, and a residential diaphragm meter, within an earthen dike. The 100 barrel tank has a plug in the heater tube and the ground surrounding the tank is stained. A pile of paraffin approximately 2 feet by 3 feet in size is directly behind the poly brine tank. The area from the paraffin north approximately 10 feet is heavily stained. Outside the dike on the southeast side there is an area approximately 3 feet by 20 feet of bare stained ground. This soil field tested with Quantabs to 8800 parts per million Chloride. Another bare spot on the northeast side outside the dike of similar size field tested for 8200 parts per million Chloride. The well is equipped with a pumping unit, rods, 2.375 inch tubing and 4.5 inch casing. Both the casing and tubing valves are in the open position. Production records end in 2014. Pictures were taken.

I spoke with Jim Posey who indicated that he would clean up the contaminated areas in and around the dike. He will excavate until the soil field tests clean. Any soils excavated will be placed within a lined vessel and transported to the Wetzel County Landfill. All manifests will be submitted to the state.

The area has been removed to specified dimensions and was found acceptable to backfill. Approximately 80 cubic yards of soil has been removed overall.

1389016174 2016-10-18 Production Wells No Violations Today I am onsite to check on the status of the cleanup efforts. Samurai Corp has backfilled all of the excavated areas and seeded the area with a seed blend approved by the landowner. New stone was placed within the tank dike and around the wellhead. The remediation activities at this location are complete.
1473185144 2016-09-06 Production Wells No Violations I am on site today to delineate the extent of clean up needed from a brine leak using Quantabs to field screen samples. A representative from Samurai Operating Company was onsite along with SLS Excavating to begin the cleanup activities. Two contaminated areas were initially identified, on the northeast and southeast sides outside of the earthen dike. The northeast areas dimensions were approximately 8 feet wide on one end and 5 feet at the other by approximately 25 feet long and 5 feet deep. The southeast area was approximately 10 feet wide on one end and 5 feet wide on the other by approximately 20 feet long and 7 feet deep. The delineated area will be excavated into a sealed roll off box and hauled to an approved landfill where it will then be tested and disposed of. Samurai will call be before backfilling for final confirmation. The excavated areas will then be backfilled with clean fill and restored back to grade then reseeded. The contaminated soil within the tank berm will be removed to approximately 2 feet then backfilled with clay covered in rock.
1473338195 2016-09-08 Production Wells No Violations On the day of my inspection I verified that the northeast contaminated area had been removed to the specified depths. SLS Excavating had removed the area to the northeast of the tank dike to the depth discussed on 9/6/16. The second excavation is underway and should be completed by 9/9/16. All non-contaminated area that come into contact with contaminated soils will be excavated to a depth of 0.5 feet.
