0262503702 | 2016-11-23 | Production Wells | No Violations | Today I am onsite to follow up on my inspection on 9/20/16 regarding no identification of any kind related to this permit number. No identification has been placed at this location and both wells on the pad are in the same state as during my last inspection. I called the Cambridge Ohio office for Ascent Resources and was contacted by the production superintendent Adam Pence. I explained that proper identification needed to be placed at the wellheads. I also mentioned that the future of these wells needed to be discussed since 34-081-2-0490 has not produced since 2010 and 34-081-2-0501 has never produced. Adam indicated that he would have identification placed at the wellhead as soon as possible and that he would contact the Ascent corporate office in Oklahoma about the future of the wells. He mentioned that they would most likely be plugging and abandoning them. |

0553522483 | 2018-11-15 | Plug / Plug Back | No Violations | On the day of my inspetion I was accompanied by Supervisor Mike Moreland. Identied ourselves to Cory Lewis from Weatherford who is overseeing plug job. Yesterday they had 2 psi on the tubing, 400 psi on the 4.5 inch diameter casing, and 1720 on the 8.8625 inch diameter casing. Bleed down for 2 hours and 8.625 still had 100 psi. This morning the 8.625 had 1361 and they bleed it down to 100 psi over 2 hours. 4.5 also has 400 on it again this morning and was bleed down. Ice storm this morning has haulted any activity. Rig is stood up over well. Plan is to try and circulate through tubing and trip more tubing in to get to bottom of well. |
0619190021 | 2019-01-03 | Plug / Plug Back | No Violations | On the day of my inspection I spoke with Cory from Weatherford. Nicks Well Plugging is now on location to plug the well. During my inspection they were tripping in the wellbore. Cory does not believe they will be setting bottom plug until Monday as they are not working over the weekend. |

No identification has been placed at this location and both wells on the pad are in the same state as during my last inspection. I called Ascent Resources production superintendent Adam Pence and left a message about placing proper identification at the wellheads. He had called and left me a message on 12/6/16 stating that proper signage would be placed on the wellheads as soon as they could make them. |
1499104536 | 2017-07-03 | Production Wells | No Violations | Today I am onsite to follow up on my inspections on 9/20/16, 11/30/16 and 12/3/17 regarding no identification of any kind related to this permit number. Ascent has placed a signage near the lease road entrance for both wells on this pad. A gauge on the flowline from this well reads 650 pounds per square inch and another on the production casing reads 400 pounds per square inch. The valve on the flowline after the gauge is in the off position. There is a metal cage surrounding the wellhead with cement barriers around the cage. There is no fence or berm around the pad. There is another shut in well on the location. Both wells are overgrown with vegetation and surrounded by orange construction fencing that has mostly fallen down or is torn. |
1507822911 | 2017-10-12 | Production Wells | No Violations | On the date of my inspection I found a gravel lease road leading to a pad with no fence or berm and two unidentified wells. Ascent has placed a signage near the lease road entrance for both wells on this pad, but the wellheads still do not have any identification tags. A gauge on the flowline from this well reads 650 pounds per square inch and another on the production casing reads 400 pounds per square inch. The valve on the flowline after the gauge is in the off position. There is a metal cage surrounding the wellhead with cement barriers around the cage. There is another shut in well on the location. Both wells are overgrown with vegetation and surrounded by orange construction fencing that has mostly fallen down or is torn. There is no production equipment associated with this well. |
1522871471 | 2018-04-04 | Production Wells | No Violations | On the date of my inspection I found a pad with no fence or berm and two unidentified wells. There is a permit sign at end of lease road but wells are not labeled. This plunger lift well is equipped with a christmas tree wellhead and a motor valve. The well is shut in. Production records are current in RBDMS, but the well has not produced since 2013. There is no production equipment associated with this well. |
1547566151 | 2019-01-15 | Plug / Plug Back | No Violations | On the day of my inspection Nicks Well Plugging is onsite. 4.5 inch diameter casing was cut at 4332 on 1/10/19. They have not pulled the 4.5 inch diameter casing yet as the 8.625 inch casing has 1100 psi on it every morning. Today they are pumping 13 ppg water based mud to try and kill the well so they can break wellhead down and pull 4.5 inch diameter casing. They are removing the wellbore fluid that is displaced by the 310 barrel of drilling fluid and putting it in a separate tank. When they began receiving mud they stopped removing fluid from the return tank. They are circulating the 310 barrels of mud and it is down to about 11 ppg. Weatherford consultant Cory is plannig on bringing the weight of the mud back up tomorrow morning. |

1548710420 | 2019-01-28 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Nicks Well Plugging pump a 30 sack plug. 4.5 inch diameter casing was shot at 4206 and pulled to 4166 feet. A 100 foot plug was displaced with 63 barrels. Cement was class A cement weighing 15.6 ppg and yield of 1.18. They are going to pull approximately 1900 feet of 4.5 casing to bring the 4.5 casing to around 2200. Cory believes the gas that they are having trouble with is around 2100. |
1549394311 | 2019-02-05 | Plug / Plug Back | No Violations | On the day of my inspection Nicks well plugging was onsite. They have 15.6 ppg fluid in the hole and wellbore is static. 4.5 inch diameter casing is at 2121. 8.625 inch diameter casing is set at 1771. The pumped a 136 sack plug of class A cement weighing 15.6 ppg and yield of 1.18. this plug was displaced with 25.9 barrels of 15.6 fluid. Plug should be from 2121-1671. This bottom of surface plug was elongated to try and cover what ever flow zone they have been dealing with. Upon tripping 4.5 inch diameter casing out of the hole the well gurlged occasionally. I witnessed well was static prior to cement job. Cory Lewis from Weatherford had nicks load the hole with 6 barrels of fluid and shut the wellbore in with zero psi. |
1549481730 | 2019-02-06 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed Nick well plugigng onsite. Cory Lewis from Weatherford said there was 370 psi on the 8.625 inch diameter casing this morning. Yesterday they pumped a 136 sack cement plug over the surface casing shoe. Plug was tagged with cable tools at 1350. I told Cory that we needed to drill the failed plug out, per Supervisor Morelands direction. Ascent does not want to do that and is going to give ODNR some other remedial proposals for consideration. |
