0718686342 | 2017-09-25 | Plug / Plug Back | No Violations | Today I am onsite to follow up on the restoration of the well site after plugging the well. Buckeye Oil Producing has removed the last 100 barrel tank and equipment from the site. Grass has started to grow over the reclaimed area. I will check back in several weeks to evaluate the final restoration of this site. |
0842067598 | 2010-09-09 | Drill / Deepen / Reopen | No Violations | Inspection revealed that Advent Drilling is at 600. No violations at this time. |

1152184262 | 2012-10-23 | Production Wells | No Violations | My inspection found a well maintained lease road leading up to three 210 barrel production tanks and a vertical separator contained in a dike. The tank battery was clearly marked with the proper signage. The well has a pump jack located on it. The site was free of any leaking oil or spills. There are no violations at this time. |
1199275927 | 2014-07-17 | Preliminary Restoration | No Violations | Well has all required equipment to produce on site. Site meets preliminary restoration requirements per ORC 1509.072(A). |
1503932972 | 2017-08-28 | Plug / Plug Back | No Violations | Today I am onsite to witness the plugging of this well. Berry Well Service is the plugging contractor, the well is owned by Buckeye Oil, and Petroset is the cement contractor. Berry Well Service had pulled the rods and tubing from the well then run 4876 feet of tubing back into the well. Petroset cleared the tubing with fresh water then pumped 30 sacks of class A, 2 percent calcium chloride, 15.6 pounds per gallon, 1.26 yield cement to set a bottom plug inside of 4.5 inch 11.6 pound per foot casing through the 2.375 inch tubing. While pumping, I witnessed fluid returns to surface. Petroset displaced with 18 barrels of water, placing the plug at approximately 4651 feet. The fluids were channeled into an earthen pit lined with plastic. Some of the spray landed on the ground surface near the pit. Buckeye used a backhoe to immediately gather the contaminated soil into a pile so the oil could not spread. The tubing was pulled after setting the plug. Berry Well Service then went down the 4.5 inch casing with ripping tools and tagged the cement plug at approximately 4470 feet. They were able to get a telegraph at approximately 4150 feet. Telegraph was lost until 3673, where they were able to rip the 4.5 inch casing. Berry Well Service pulled one 44 foot joint and shut down for the day. |
1504023253 | 2017-08-29 | Plug / Plug Back | No Violations | Today I am onsite to witness the continued plugging efforts for this well. Petroset circulated with fresh water then set a 200 foot rip plug from 3629 feet to 3429 feet. They pumped 50 sacks of class A, 15.6 pound per gallon, 2 percent calcium chloride, 1.26 yield cement and displaced with 53 barrels of fresh water. Berry Well Service then pulled 45 joints of 4.5 inch, 11.6 pound per foot casing. At 44 feet per joint, the surface casing plug will be set through 1649 feet of 4.5 inch casing from 1649 feet to 1414 feet. Petroset established circulation with fresh water then pumped 60 sacks of class A, 15.6 pound per gallon, 2 percent calcium chloride, 1.26 yield cement and displaced with 22 barrels of fresh water. Berry Well service then pulled 33 joints of 4.5 inch casing to set the 132 foot surface plug. For the surface plug, Petroset established circulation with fresh water then pumped 55 sacks of class A, 15.6 pound per gallon, 2 percent calcium chloride, 1.26 yield cement. I witnessed good cement return to surface. Berry Well Service pulled the remaining 4.5 inch casing and Petroset topped out the 8.625 inch casing with cement. Buckeye Oil will cut the casing off approximately 30 inches below ground surface and weld an identification plate to it. They plan to remove their equipment and reclaim the area within 1 week, and will notify me once the work is complete. |
1505136545 | 2017-09-11 | Plug / Plug Back | No Violations | I am on site today to follow up on the plugging of this well. Buckeye Oil Producing is currently hauling a small pile (approximately 5 tons) of contaminated soil from plugging operations off to APEX landfill. They will provide the waste manifests when they receive them. Buckeye plans to finish reclaiming the site this week. |
1505303619 | 2017-09-13 | Plug / Plug Back | No Violations | I am onsite today to follow up on the restoration of this well site following the well being plugged. Buckeye Oil Producing has re-graded the area. They have seeded it and placed straw over the area. Currently there is still one 100 barrel tank that must be removed. They also have a backhoe, bulldozer and dump truck on the site. The operations should be completed by 9/14/17. |
1513692636 | 2017-12-19 | Final Restoration | No Violations | I am on site today to perform a final restoration inspection for this permit number. This well was plugged on 8/29/17. All oil and gas associated equipment has been removed from the location. The area has been re-graded and healthy grass is growing. The final restoration for this permit number is now considered to be complete per ORC 1509.072(B). |
1786242398 | 2010-10-12 | Production Wells | No Violations | Inspection revealed that the well has been fractured, completed, and placed into production. The operator is pumping the well into a frac tank at this time. No violations at this time. |
2002753184 | 2010-09-08 | Drill / Deepen / Reopen | No Violations | Inspection revealed that Advent Drilling is moving on location. Inspection of the drilling pits revealed no violations at this time. |
2042334598 | 2016-08-11 | Production Wells | No Violations | On the date of my inspection I found the tank battery to include 3 properly identified 100 barrel tanks, a vertical separator, a diaphragm meter, and a drip. The valve from the well to the separator is shut off. The well is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. The tubing and casing valves are in the open position. Production records are current. |
