0270647812 | 2013-10-07 | Preliminary Restoration | No Violations | I was onsite to pass preliminary restoration for this well. Drilling and fracking equipment have been removed from the location and preliminary restoration can now be passed. |
0392560305 | 2012-10-24 | Production Wells | No Violations | My inspection found that the lease road was well maintained with a locked gate. The well pad was fenced and contained two wells with assembled christmas tree wellheads onsite. The wells were surrounded by steel fencing and cement barriers. The well pad also consisted of a six 400 barrel tank battery with steel dike in place with separators and production equipment. A crew was onsite painting the tank battery during the inspection. There are no violations at this time. |

1456766581 | 2016-02-29 | Production Wells | No Violations | My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #5H well inactive with the gate valves chained and padlocked. No annular pressure gauge was attached. Concrete barriers were in place around the wellhead. Well identification number was found on the well and on signage at entrance to access road. A freshwater impoundment adjacent to the pad is fenced and secured, with identification signage attached to the fence. Approximately 5 feet of freeboard was observed. The equipment on the pad consists of the following:
6 400 bbl. storage tanks contained in steel dike. 2 inactive wells. 1 VRU. 2 Upright Sand Separators. 2 GPUs. |
1465842250 | 2016-06-06 | Drill / Deepen / Reopen | No Violations | I am on site today to witness the cementing of surface casing. Nomac Rig 74 drilled a 24 inch diameter hole to a depth of 418 feet then ran 416 feet of 20.00 inch diameter 94 lbs. /ft. surface casing. The shoe was set at a depth of 416 feet. Schlumberger Cementing Services established circulation then used 842 sacks of Class A 15.6 pound/1.2 yield cement to bring cement returns to surface. While cement was coming to surface the wiper plug landed; I witness Schlumberger pumping 500 PSI over pump pressure and the casing proved integrity. Approximately 60 bbl. of good cement returns was brought to surface. |

1481784182 | 2013-10-08 | Preliminary Restoration | No Violations | I was onsite to pass preliminary restoration for this well. Drilling and fracking equipment have been removed from the location and preliminary restoration can now be passed. |
1531694476 | 2018-03-22 | Production Wells | No Violations | On the day of my inspection I found this pad containing two properly labeled producing wellheads. Production Equipment includes two gas production units, two sand seperators, and one 400 barrel steel tank, inside its own steel containment. Surface annular pressure was 0 PSI and interemdiate pressure was 320 PSI. Pad is stable and vegetated. |
1814856197 | 2012-06-21 | Production Wells | No Violations | DNR Inspector Rick Anderson and I were on site at DNR Supervisor Gene Chinis request. He received a phone call from Ed Hasberger who stated that he had concerns about trucks (marked brine) either hauling or dumping water into the frac impoundment on the DeNoon site. Our inspection revealed no visible evidence of any recent water truck activity at the site. There were no fresh tracks in the stone in front of the freshwater frac impoundment and no signs of any sediment being disturbed in the impoundment . There are no visible signs of any water being dumped into the freshwater frac impoundment. Chesapeake is drilling the Kirk well on State Route 43 north of this site and there is a possibilty they needed some water to drill with and had a couple of water trucks haul some water from the frac impoundment. I contacted Supervisor Chini with our findings and he advised that he would contact Mr. Hasberger. |
1872874699 | 2014-09-18 | Production Wells | Violations Noted | On this date the pad contents include a tank battery with 4 400bbl tanks in containment and production equipment. Well is equipped with a large texas style wellhead. No pressures are shown on the tubing or casing gauges. There is a Chesapeake freshwater impoundment adjacent to the pad that appears stable. No production numbers have been submitted for this well. |
