0807772963 2018-03-23 Production Wells No Violations On the day of my inspection I found this pad containg two properly labeled plunger lift wells. A third cellar is set on this pad but does not appear to have a conductor casing in it. Surface and intermediate annular pressures read 0 psi. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tank, within their own steel containment. Production records are up to date.
0808980085 2012-10-24 Production Wells No Violations My inspection found that the lease road was well maintained with a locked gate. The well pad was fenced and contained two wells with assembled christmas tree wellheads onsite. The wells were surrounded by steel fencing and cement barriers. The pad also still consists of sound reduction barriers facing the residential structures surrounding the pad. There are no violations at this time.
1080377175 2015-03-18 Production Wells No Violations On the date of inspection I am on site with SE Regional Manager Brent Bear to inspect the well for placement into Temporary Inactive Status due to a lack of pipeline infrastructure. At this time the well is only equipped with a well head, a Christmas tree has not yet been installed. There is no production equipment on site at this time. There are two wells located at this well pad and there is no identification. Both cellars are filled with fluid which will be required to be pumped out. I contacted the Chesapeake field office and spoke with Manny Johnson. Mr. Johnson informed me that he would have the local well tender come to the site and address the issues as soon as possible. After Chesapeake has corrected the above issues, the well can pass field inspection for Temporary Inactive Status.
1150724330 2013-10-29 Preliminary Restoration No Violations I was on location to pass preliminary restoration on this well. At this time all of the drilling equipment has been removed off location and there is currently only a wellhead on location at this time. Preliminary restoration can be passed at this time.
1251920331 2014-09-16 Production Wells Violations Noted Well was drilled in the summer of 2012. No production records for the well are available. Well is shut in, has no production equipment onsite, and does not have the christmas tree valve assembly on the wellhead. No pipeline to well yet.

Our inspection revealed that the rig had the drill string out of the hole (depth of 8144). The rig consultant, Danny Lambert, advised that they should be going back into the hole later today and should start drilling sometime tonight.

1490791287 2017-03-29 Production Wells No Violations This is a follow up to the inspection performed on 3/9/17 where I contacted Jody Jones with Chesapeake about the contamination in the cellar of this well. A new chloride test using Quantabs indicates the fluid in the cellar has a concentration of approximately 6078 parts per million. There is still a sheen on top of the fluid within the cellar. None of the contamination appears to have been removed or cleaned up.

1624229234 2017-04-06 Production Wells No Violations Today I am following up on an inspection on 3/29. I met with the Chesapeake production manager for Jefferson County. He indicated that the cellar for this well had been flushed out with approximately 100 barrels of fresh water. Quan tabs indicated that chloride levels in the cellar are now approximately 250 parts per million, which is within the acceptable level. The concentration will continue to be monitored on a weekly basis by Chesapeake.
