0107517000 2015-03-18 Production Wells No Violations On the date of inspection I am on site with SE Regional Manager Brent Bear to inspect the well for placement into Temporary Inactive Status due to a lack of pipeline infrastructure. At this time the well is only equipped with a well head, a Christmas tree has not yet been installed. There is no production equipment on site at this time. There are two wells located at this well pad and there is no identification. Both cellars are filled with fluid which will be required to be pumped out. I contacted the Chesapeake field office and spoke with Manny Johnson. Mr. Johnson informed me that he would have the local well tender come to the site and address the issues as soon as possible. After Chesapeake has corrected the above issues, the well can pass field inspection for Temporary Inactive Status.
0259421410 2012-07-10 Drill / Deepen / Reopen No Violations DNR Inspector Shawn Murphy and I were on site to check the status of Patterson Drilling Rig # 329. Our inpection revealed that the rig was drilling 7388 and should be landing their curve sometime Thursday night. We talked with the rig consultant ( Danny Lambert) and he advised after they were finished on this pad, they should be moving to the Slates Pad for 5 or 6 wells. Our inspection of the well site revealed no violations at this time.
0306373102 2014-03-05 Preliminary Restoration No Violations I was at the site to conduct a preliminary restoration inspection. During my inspection, I found the wellhead to be equipped with a Christmas tree valve assembly with all required pressure gauges and valves. The wellhead is in good condition, and is protected from vehicles by concrete barriers. The location has a graveled lease road with a gate, and the pad is fenced and graveled. The pad contains vertical separators and other production equipment. Six 400 bbl production tanks are situated within a coated steel SPCC containment. The erosion controls are in place and functioning, and vegetation is well established. Preliminary restoration is passed.
0350771056 2012-10-24 Production Wells No Violations My inspection found that the lease road was well maintained with a locked gate. The well pad was fenced and contained two wells with assembled christmas tree wellheads onsite. The wells were surrounded by steel fencing and cement barriers. The pad also still consists of sound reduction barriers facing the residential structures surrounding the pad. There are no violations at this time.

1341287760 2012-07-02 Drill / Deepen / Reopen No Violations I arrived on site at approximately 8:20pm to witness the cementing of the 13 3/8 surface casing. Upon arrival I observed that Patterson Drilling Rig # 329 was running the last joint of casing into the ground and was washing the casing down the hole. There was fill-up (cuttings) in the bottom of the hole. The rig finally got the casing washed down to 576 and circulated bottoms up before installing and hooking up O-Tex Cementings cement head. O-TEX cemented the 13 3/8 casing with 510 sacks of Class H cement. They pumped 81 barrels of displacement and landed the plug. Approximately 16 barrels of cement returns to surface. Leave the well site at 3:54am.
1341607051 2012-07-06 Drill / Deepen / Reopen No Violations I spoke with Chris (Rig Consultant) via cell phone at 7:34am (07:34hrs). He notified me that they should be ready to cement around 12:30pm (12:30hrs) today. I called the drilling rig at 12:16pm (12:16hrs) and they notified me that they would be ready to start pumping cement around 3:00pm (15:00hrs) today. I arrived on site at 3:10pm (13:10hrs) and observed that they were still running the 9 5/8 casing. I went to the consultant trailer and obtained the total depth and casing information for this section of the well. Patterson Rig # 329 drilled this section of the well on air and reached total depth of this section at 2135. They ran 2127 of 9 5/8 casing (40# J-55 LT&C) and landed the casing in the Weatherford multi-bowl wellhead. NOTE: We had to wait on Schlumberger to arrive on location. They cemented the 9 5/8 casing with 864 sacks of Class A cement. There was good circulation throughout the job and there was approximately 60 barrels of cement returns to surface. I departed the well site at 12:05am (00:05hrs) on July 7.
1382686985 2017-04-06 Production Wells No Violations Today I am following up on an inspection on 3/29. I met with the Chesapeake production manager for Jefferson County. He indicated that the cellar for this permit number had been drained at the same time the cellar of permit 3408120519 had been flushed out. Previous chloride tests indicated a concentration of approximately 300 parts per million. The water level in the cellar could not be tested today as the cellar has not filled back up with water.

1594941656 2018-03-23 Production Wells No Violations On the day of my inspection I found this pad containg two properly labeled plunger lift wells. A third cellar is set on this pad but does not appear to have a conductor casing in it. Surface and intermediate annular pressures read 0 psi. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tank, within their own steel containment. Production records are up to date.
