0201809561 | 2013-07-10 | Production Wells | No Violations | I was onsite to check the status of the production wells on this location. The location has the production facilities completely finished and the wells are being sold into a poly line, causing the wells to be choked back from full production at this time. I also checked the annulus gauges of the wells and this well showed 225 psi on the 13 3/8 annulus. There are no violations at this time. |
0319176030 | 2013-11-12 | Drill / Deepen / Reopen | No Violations | Currently they are preparing to test the casing liner that was set in the well in order to cover the perforations done to squeeze the well and stop backside pressure buildups. They tested the liners to 11,000psi and there was no pressure falloff. They plan to begin the first stage of the hydraulic fracturing Saturday. |

0626882422 | 2013-11-16 | Drill / Deepen / Reopen | No Violations | Today I witnessed the first stage of the NAC 3h-20 Frac job. The stage was successful without any issues. Pressures on all the annuli were monitored without any increases. The injection pressures were kept below 10,600psi (casing liners tested to 11,000psi) and injection rates at 80bpm. The job did have to be shut down twice due to them running out of water in their frac tanks. Normally they can heat 80bpm of water to 80F for the jobs but they had to run 100F water in this job so that the casing liners would not shrink due to the colder temperatures and compromise borehole integrity. They could only heat at 46bpm and they also used a new sand mixing system to reduce dust onsite which increased water flow line length and limited their withdrawal limits. The issues were fixed for the following stages. They plan on being down with the 16 stages by Sunday. |
0638989115 | 2013-12-09 | Drill / Deepen / Reopen | No Violations | I was on location to check the status of the rigging down of the completions crew. Currently the flare stack is being rigged down and moved off location. The frac wellhead is still on the well but the well has been shut in. The well has been turned over to production personnel at this time. There was about 600psi of pressure seen on the backside of the 5.5 casing according to the flow back crew, but it was bleed off and did not build back up before my arrival. There are no violations at the time of inspection. |
0656206684 | 2013-11-21 | Drill / Deepen / Reopen | No Violations | The last stage of the NAC hydraulic fracturing has been complete. The 15 stages of the well job went well and they are currently preparing to drill out the plugs on the well with the coiled tubing unit. They will be moving the operation to the Smithfield well in Jefferson County following the completion of this job. |

0850247331 | 2012-08-14 | Plug / Plug Back | No Violations | I was on site because H&P Drilling Rig # 287 lost the 12 1/4 hammer down the hole. They were drilling at 5546 when they lost the hammer. They made three attempts to fish the hammer out of the hole, but lost it every time they started back out of the hole. The top of the hammer is at 5541 so they tripped drill pipe back into the hole to the top of the hammer. Halliburton pumped a 483 sack plug (densecem 16.8 density and 1.02 yield) through the 5 drill pipe. Rig pulled 10 stands and pumped a ball through the drill pipe to clean the iside of the drill pipe. They advised that they would use this as a kick-plug and kick out around the lost hammer. My inspection revealed no violations at this time. |
0907312205 | 2014-07-24 | Production Wells | No Violations | Met with AEU representatives Kevin Eshbaugh and Corey McNutt to gain access to site. This well is currently producing. AEU reps stated that they are unable to flow the wells on this pad simultaniously due to pipeline restrictions and that they are alternating production on the wells. Large texas style wellhead with 1 400bbl tank, 3 phase seperator, line heater, and dehydrator are onsite. |

1089299675 | 2013-10-11 | Preliminary Restoration | No Violations | I was on location to pass preliminary restoration for this well. The well has had a squeeze performed on it to address an annular gas migration problem for the well. The well is currently waiting to be hydraulically fractured but is on a well pad with several other wells that have had preliminary restoration approved. This well, if and when is completed will use the existing equipment on location. At this time preliminary restoration can be passed. |
1176437808 | 2012-11-02 | Drill / Deepen / Reopen | No Violations | I was onsite to check the status of the remediation of the annulus pressure problems of the well. A service rig was onsite and was currently in the act of being erected. I talked with Mr. Marcus Attaway, completions supervisor, regarding the schedule. The plan is to set a packer in the 5 1/2 casing today. After changing the wellhead iron, installing the BOPs for the workover rig and completing the pressure testing required for those processes tubing will be ran into the well to spot a cement plug above the packer. The cementing is set to start tomorrow noon to mid-day. |

1343793770 | 2012-07-31 | Drill / Deepen / Reopen | No Violations | I was on site to check the status of H&P Drilling Rig. My inspection revealed that they are still in the process of rigging up. I met with Ryan Gonzales and Armando Rodriguez (safety personel). Armando Rodriguez gave me Hess safety training and we discussed Hess safety policies/ procedures, personal protective equipment (PPEs), and sign in and out protocol. I advised him that they might encounter hydrogen sulfide gas while drilling this well. He advised that they have a guy on site that is qualified to train / instruct on issues dealing with hydrogen sulfide. They also advised that they would like all visitors to sign in at the guard house and the safety building prior to going on site. They advised me that they should be spudded sometime tomorrow morning. My inspection revealed no violations at this time. |
1343987807 | 2012-08-02 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 surface casing. My inspection revealed that H&P Rig # 287 was still running the 20 casing when I arrived on site. They were having trouble getting the casing to thread together so the decided that they should weld each joint after they got them threaded into the collar. They had to call a welder and he advised that it would be at least an hour before he could arrive on location. I talked with Bill Calabreeze (Rig Consultant) and we figured it would be at least 4 or 5 hours before they would have casing ran into the wellbore. I advised him that I was going to leave the site and return later tonight. I also advised him that I would call Ruben Perez (Night Rig Consultant) and advise him to call me when they got casing on bottom. I departed the site at approximately 10:00pm and went to my residence. |
1352316982 | 2012-11-05 | Drill / Deepen / Reopen | No Violations | On 11/5/2012, I arrived on location at 8:00PM with ODNR Inspector Fred Romeo to witness the cement squeeze procedure on the CROSS CREEK NAC 3H-20. Halliburton was to be on location to start the process by 9:00 PM. When we arrived on location at 8:20 PM they were still running 8200 of 2 7/8 tubing, they finished running tubing at 10:00 PM, at that time Marcus Attaway Company Representative from Hess informed us that Halliburton would not be on location for at least 4 to 5 hours, a miss communication at Halliburton Office. I called Deputy Chief Robert Worstall about the time, it is 102 miles from my house to the location, he ask if I could stay to witness the job I agreed to do so. When Halliburton arrived they began to rig up there equipment start mixing there resin mixture and cement, Safety meeting, job procedure meeting . The 1st stage was completed at 12:00PM hrs. I returned home at 2:00Pm. See ODNR Inspector Fred Romeos job report for summary. |
