0545206699 2017-02-28 Plug / Plug Back No Violations Today I am onsite to follow up on my inspection on 12/27/2016. The cellar of this abandoned well has still not been backfilled with gravel. I will contact Chesapeake again about this matter.
0598839537 2014-09-16 Drill / Deepen / Reopen Violations Noted Conductor has been set on this well. Further drilling of the well was never completed. Drilling permit expired in May of 2014. Chesapeake plans of plugging the well according to emails from Steve Opritza.
1693042611 2017-06-13 Final Restoration No Violations I am on site today to perform a final restoration inspection for this permit number. This well was plugged on 10/17/14. The cellar ring is still in place and will be removed when the pad itself is reclaimed after the other wells have been plugged. The cellar is not filled with stone but has a metal grate covering the opening. A cattle fence is around the cellar and all 3 wells on the pad are surrounded by concrete barriers. The well is located on a pad with two other producing wells. No land reclamation will be required for this permit until all wells on the pad have been plugged and abandoned. The soil stockpile from the pad is still preserved for reclamation use. The final restoration for this permit number is now considered to be complete per ORC 1509.072(B).
1739611354 2014-10-17 Plug / Plug Back No Violations Conductor is the only casing set in this well. 20 conductor pipe set to 110. 12 of cement was left in the bottom of casing at the time of cementing the conductor. Operator placed 7 yds of readymix concrete with a 3000 psi rating, into the casing and then welded a plate with API number to the top of the casing. 7 deep cellar is still open and intact. Casing is cut off 6.5 below ground level. This well is on a pad with 2 other wells that were drilled to total depth and cannot be passed for final restoration. Mousehole for this well has also been cemented back to surface.
