0172388859 2012-07-10 Drill / Deepen / Reopen No Violations DNR Inspector Shawn Murphy and I were on site to check the staus of Patterson Drilling Rig # 154. Our inspection revealed that they have the drilling rig moved onto location, the derrick in the air, and they are finishing rigging up the Mathena System. They stated that if everything went according to plan, they should spud the well sometime tomorrow morning. No violations were noted while we were conducting our inspection.
0248410263 2013-10-11 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The frac has been completed and the drilling equipnment has been removed from the location. At this time preliminary restoration can be passed.
0326119980 2012-07-31 Drill / Deepen / Reopen No Violations I was on site to check the status of Patterson Drilling Rig # 154. My inspection revealed that the drilling rig has drilled to a total depth of 15034 and are laying down the drill string at this time. They have approximately 7914 of the drill string pulled out of the hole and laid down on the pipe racks. They should be ready to start running the 5 1/2 production casing later today and be ready to cement the casing sometime tomorrow. My inspection revealed no violations at this time.
0693707133 2016-10-04 Production Wells No Violations On the date of my inspection I found a well maintained lease road with a fence and gate. The pad has 1 well and no production equipment. Pressure on the production casing is at zero pounds per square inch. The pad is surrounded by healthy vegetation. The wellhead is equipped with a christmas tree valve that is shut in. According to records, the well was completed on 8/30/2012 and is currently listed as temporarily inactive.
0718750176 2015-05-26 Drill / Deepen / Reopen No Violations On the date of inspection I am on site to inspect the well for Temporary Inactive Status due to a lack of pipeline infrastructure. The well site is fenced and gated with locks on both gates. There are concrete barriers around the well head and cellar. The well head is equipped with a Christmas tree with chains wrapped through the valves. There has been a hole cut into the cellar approximately 5 feet from bottom. The hole is approximately 1 foot wide by 6 inches high and has allowed infiltration of mud/gravel from the well pad. There is no production equipment on site. Identification is present at the roadway entrance. I spoke with Jody Jones, Regulatory Analyst, with Chesapeake and he informed me that identification signs had been ordered to place at the well head.
1074181806 2013-10-11 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The frac has been completed and the drilling equipnment has been removed from the location. At this time preliminary restoration can be passed.
1342428058 2012-07-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. I arrived on site at 03:45am and found that Patterson Drilling Rig #154 had reached a total depth of 1865 and was still running the 9 5/8 intermediate casing. I had spoke with the Drilling Rig Consultant (Lawrence), at 01:02 am, and he advised me that they should be finished running casing and ready to cement at around 3:00 to 3:30 am. NOTE: The rig had trouble getting the casing landed on bottom. They tried to wash casing to bottom but the casing wouldnt go any deeper. They went over the original casing tally and found that they had an extra 10 joint of casing in the hole. They pulled the landing joint and the 10 joint of casing. They ran the landing joint back in the well and landed the casing at 1865. Halliburton cemented the casing with 700 sacks of Class A cement with 12 % Salt and 1/4 pound per sack of Floseal. Well circulated throughout the entire job and approximately 30 barrels of cement circulated to surface.
1342579522 2012-07-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. Patterson Drilling Rig #154 had drilled the surface hole to 650 and found that they were in a sand type formation with the mud loggers indicating a show of gas. The drilling rig consultant (Tim Allen) advised that they were concerned with the gas and didnt feel comfortable cementing the surface casing at that depth, with the fresh water aquifers and the gas behind the same string of casing. They had decided to plug the well back from 650 to 400 with Schlumberger pumping the 250 plug. They let the plug set for 8 hours and then they drilled the top off of the plug to a depth of 480 (surface hole total depth) . The drilling rig ran 480 of 13 3/8 casing and Schlumberger cemented the casing with a total of 528 sacks of cement. There was good circulation throughout the entire job and there was 47 barrels of cement circulated to surface.
1363636786 2013-03-21 Production Wells No Violations My inspection found a well maintained lease road with a gate. The well pad is surrounded by fencing, sound barriers and has sediment control netting surrounding it. The christmas tree wellhead is assembled and shows pressure on the production casing of around 1800psi. There are no violations at this time.
1364912706 2013-04-02 Production Wells No Violations My inspection found a well maintained lease road with a gate. The well pad is surrounded by fencing, sound barriers and has sediment control netting surrounding it. The christmas tree well head is on the well. There is no production equipment on location at this time, the well is shut in. There are no violations at this time.
1489576186 2012-10-15 Production Wells No Violations My inspection found a well maintained lease road with a gate. The well pad is surrounded by fencing, sound barriers and has sediment control netting surrounding it. The well head is on the pad and the well is waiting to be hydraulically fractured. No violations at this time.

1672514228 2012-08-02 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 5 1/2 production casing. My inspection revealed that Patterson Rig # 154 has ran 14,990 of 5 1/2 production casing and are circulating on the casing at this time. After they finished circulating, they rigged down the CRT tool and rigged up the Blackhawk Cement Tool. I had to leave the site before they started cementing the 5 1/2 casing because I had to go to permit number 528 in Jefferson County for a surface casing job. Before I left the site, Tim Allen (Rig Consultant) advised that they are going to move the rig to the Fowler site in Columbiana County. My inspection revealed no violations at this time.
