0098651924 | 2016-10-04 | Production Wells | No Violations | On the date of my inspection I found a well maintained lease road with a fence and gate. The pad has 1 well and no production equipment. Pressure on the production casing is at zero pounds per square inch. The pad is surrounded by healthy vegetation. The wellhead is equipped with a christmas tree valve that is shut in. According to records, total depth was reached on 5/14/2014. |

0203889527 | 2014-05-19 | Drill / Deepen / Reopen | No Violations | I was on location with Inspector Dylan Pendleton to check the status of Nomac 70 rig move. The rig is off location and Hodges Trucking is currently removing the trailers from location. The Great Plains frac tanks are still on location and the rig containment is still in place. Nomac 70 is now on its way to Columbiana County. There are no violations at this time. |
0476291952 | 2014-05-02 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the surface casing. 846 of new 13 3/8 J-55 casing was installed with 6 centralizers. The hole drilled easy with no losses or gains. The hole was filled with water, the casing installed and was cemented by O-tex. 162bbls of cement, cement scaled at 15.6ppg, were pumped and displaced with 123.5bbls of freshwater. Returns were taking in the cellar and 30bbls of good cement were returned to surface. The floats held at 883psi and .5bbl flowed back to the pump truck. They will be drilling to 2100 to set intermediate casing after waiting on cement 8 hours. There are no violations at this time. |
0761407395 | 2014-07-16 | Preliminary Restoration | No Violations | Well has been drilled with no production equipment on site. Site meets preliminary restoration requirements per ORC 1509.072(A). |
0762082328 | 2016-11-14 | Production Wells | No Violations | I am onsite today to perform an inspection at the request of Chesapeake Exploration LLC for a temporary inactive status for this permit number. I found a well maintained lease road with a fence and gate. The pad has 1 well and no production equipment. Pressure on the production casing is at zero pounds per square inch. The pad is surrounded by healthy vegetation. The wellhead is equipped with a christmas tree valve. The pressure gauge shows 0 pounds per square inch. There is no production equipment onsite. Chesapeake has requested a status change for one year until a pipeline can be ran for the well to be tied into. |

1398044719 | 2014-04-18 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 20 conductor casing. The 26 hole was drilled to 120 and the 20 casing was welded and installed in the hole. The well was cemented with 73.6 bbl of cement slurry (from 350 sacks of cement with a yield of 1.18 ft3 per sack). Circulation was established but cement did not return to surface, and stayed 30 below surface. 21 bbl of cement slurry (from 100 sacks of cement with a yield of 1.18 ft3 per sack) were used to top off the hole, but the cement still stayed at 30 below surface due to a loss flow zone. A mixing truck was brought in with 7 cubic yards of ready mix cement which was used to successfully top off the hole. Construction of the pad is not yet complete, but is planned to be finished before Wednesday of next week (4-23-2014) as the drilling rig is scheduled to come in Thursday (4-24-2014). |
1398280880 | 2014-04-23 | Surface Facility Construction | No Violations | During my inspection I found the conductor to have been grouted from the top with red mix and surrounded by fencing. The location is partially completed at this time. The pad has been stabilized and graded with stone. The berm on the pad have been built up and seeded. The fence structure has been installed but the chain link has not been installed at this time. The gate to the entry road also has not been installed yet. Construction equipment is currently being loaded to be brought off location. Plastic is going to be laid this week and the rig will be moving in when their current well is done being drilled. There are no violations at this time. |
1399385133 | 2014-05-05 | Drill / Deepen / Reopen | No Violations | I was on location with Inspector Dylan Pendleton to pick up the cement tickets from the previous cement jobs and to tour the rig. Currently the rig is drilling on air at 5385 and is running a clean up cycle. They will continue drilling ahead to their mud up point were they will trip out of the hole and load the hole with oil based mud. |
1465184719 | 2014-03-27 | Drill / Deepen / Reopen | No Violations | I went to the site to review the construction process of the drilling pad. The entry was started with rock, and the access road was cut, but not rocked. The drilling pad was cut and currently topsoil is being moved to level the location. The trees and brush on location have been cut and chipped. The location is a field so minimal tree and topsoil removal should be required to build the drilling location. |

1815882426 | 2014-10-01 | Drill / Deepen / Reopen | Violations Noted | Upon my arrival onsite I found a frac crew moving off location. I was informed by company man that the operation moved onto location on 9-22, they started down the hole on the 25th, and they finished up their operations this morning, 10-1-14. I asked him if he had contacted any inspectors prior to coming on location and he believed that the office did that. I put a call in to Joe Baker in Lousiville, Ohio field office but did not reach him. Also contacted ODNR Inspector Fred Romeo, who is listed on the permit as the point of contact, and he was not notified. Company man believes a work over rig will be on location early next week. Inspector is to be notified of well stimulation operations as listed on the permit. |

2074801758 | 2019-04-17 | Production Wells | No Violations | On the day of my inspection I found this pad to contain one producing well. Production equipment includes one GPU, one sand seperator, and one 400 barrel steel tank in its own containment. Wells is producing on natural reservoir pressure and operated by a motor valve. 13.375 x 9.625 annular pressure is zero and 9.625 x 5.5 annular pressure is zero. Pad is stable and vegetated. |
