0040330313 2015-05-10 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor pipe. I was informed by Chesapeake consultant Brett Farnsworth that Rocky Mountain Drilling had spudded on 5-9-15 and drilled a 36 inch diameter hole to a depth of 120 feet. Chesapeake has elected to set a 30 inch diameter, 140lbs/ft, 29.25 inch ID rather than the 26 inch conductor that was permitted in anticipation of a mine string. The 30 inch conductor was set on bottom. I field titrated mix/displacement water with a Quantab and found chlorides to be at a level of 56ppm. Express Energy Services pumped 330sk of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl2, and 0.50lbs.sk of cello flake. Express displaced cement with 91bbl of fresh water. 30bbl of cement returned to surface and stayed at surface while I was on site.
0485350716 2018-03-22 Drill / Deepen / Reopen No Violations On the day of my inspection I found this pad to contain one properly labeled producing well and one unlabled conductor casing, which is this permit. Cannot do a preliminary restoration on this permit as it has not been drilled to total depth. Permit 585 is in production and a preliminary restoration was completed for that permit.
0893346072 2016-03-10 Drill / Deepen / Reopen No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #8 partially drilled with only 30 inch conductor in place, ascertained from the previous MRI Inspection Report for this well dated 5/10/2015. Tubular gates and concrete barriers were in place around the cellar. Well identification number for this well was not found on the well nor on signage at entrance to access road. The equipment on the pad consists of the following:

2 400 bbl. production tanks contained in a metal dike. 1 Shut-in well. 1 well with conductor in place. 1 Upright Sand Separator. 1 GPU.
