0220436891 2016-12-08 Completion Testing No Violations On the date of my inspection I found Keane Frac onsite. Two wells are being hydraulically fractured. According to Ascent Resources they are approximately half way done with the stimulation of both wells.
0230620885 2014-12-09 Casing String Witnessed No Violations On the date of inspection I am on site to witness the surface string cement job. I was informed by American Energy Utica consultant, Brandon Gwin, that the Patterson 294 rig had drilled a 26 inch diameter hole to a depth of 624 feet. The Rotary Kelly Bushing is at 28 feet above ground surface. All depths will reflect this height. Wyoming Casing Service set 18.625 inch diameter, 87.50lbs/ft casing on bottom. O-Tex Pumping, LLC pumped 780sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl2, 0.4% C-41P, and 0.25lbs/sk of cello flake. O-Tex displaced cement with 178bbl of fresh water. Plug was bumped, floats held, 17bbl of cement returned to surface.
0256829784 2016-11-23 Completion Testing No Violations On the date of my inspection I found Ascent Resources setting up the pad to hydraulically fracture both the 1H and 3H. I spoke with Darcy from Ascent, who indicated the frac jobs should be starting the week of 11/28/16 and that he would notify me before the job started. Ascent is currently setting up containment and water lines. Any hazardous materials onsite at this time were within secondary containment. The 5H well is currently shut in and will remain that way until the frac jobs are complete.
0281573362 2016-09-20 Production Wells No Violations On the date of my inspection I found the pad to be adjacent to the road with a barbwire fence and gate. The pad has 2 properly identified producing wells, 2 unmarked newly completed wells that are not currently producing, and 2 cellars with conductors, 2 sand separators, 2 vertical separators, 2 glycol dehydration units, and 2 shared 400 barrel tanks within a metal dike. The tank battery is within a different fenced area adjacent to the pad containing the wells. The pad is surrounded by an earthen dike covered by healthy vegetation. Currently workover rig equipment is being removed from the site. The wellhead is equipped with the lower portion of a christmas tree valve. No pressure valves have been installed on the well, this well is not producing.
0635673154 2016-12-16 Completion Testing No Violations On the date of my inspection I found Deep Well Services rigging up drill out equipment onsite. According to Ascent Resources they should be rigged up within a day or so to begin drilling out the wells.
0674256393 2015-03-10 Drill / Deepen / Reopen No Violations On the date of inspection I visited the site to find out if the Patterson 294 rig was still present. The rig was moved out and the liner was still in the process of being removed. No one was on site during the duration of my inspection. The well pad was not currently equipped with a gate or a fence. The gated fence to the production equipment was also left open.
0853952140 2017-01-03 Production Wells No Violations On the date of my inspection I found an Ascent subcontractor onsite working on connecting the two wells that were hydraulically fractured last month. I spoke with a company representative who said they should be finished with their operations either today or tomorrow. He was unsure of when flowback would begin on these wells.
1027070143 2018-01-18 Preliminary Restoration No Violations On the day of my inspection I found the pad has 4 properly labeled producing wells equipped with christmas tree style wellheads, 4 sand separators, 4 vertical separators, 4 line heaters and pipeline equipment. On a separate fenced area north of the well pad there are 2 shared 500 barrel tanks within a metal dike, a large vertical separator, and a gas production unit. The pad also has two steel plates covering cellars south of the producing wells. No pressure on surface or intermediate casing gauges. Production records are current throug 3rd quarter 2017. Site meet requirements of ORC 1509.072A for prelininary restoration.
1190291626 2014-11-08 Casing String Witnessed No Violations On the date of inspection I am on site to witness the conductor cement job. I was informed by American Energy Utica consultant, Jerry Bright, that Precision Rathole Inc had drilled a 36 inch diameter hole to a depth of 100 feet. Precision ran 100 feet of 30 inch diameter, 118.64lbs.ft casing to bottom. Express Energy Services pumped fresh water ahead until fluid began to rise in the annular space, then pumped 275sk of Class A Cement, 2% CaCl, 0.50lbs/sk of celloflake, 15.6ppg, 1.18 yield. Express displaced cement with 75bbl of fresh water, 10bbl of cement returned to surface and stayed at surface while I was on site.

1419987994 2014-12-30 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica consultant, Cecil Lovell, that the Patterson 294 rig was currently picking its Bottom Hole Assembly to begin drilling the 8.5 inch diameter hole. The 9.625 inch diameter second intermediate had been cemented the previous weekend. The rig was currently at a depth of 8793 feet.
