Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0634777600 2014-11-25 Casing String Witnessed No Violations Precision Drilling drilled 100 of 36 hole and set 100 of 30 casing. Express Services cemented it with 275 sacks of Class A cement. 7 bbls of good cement returned to the surface. See the casing ticket for the details.
1082468927 2015-05-26 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1085757811 2017-08-22 Production Wells No Violations
1418236077 2014-12-10 Casing String Witnessed No Violations On the date of inspection I am on site to witness the 18.625 inch casing string. I was informed by American Energy Utica (AEU) drilling superintendent, John Melville (Larry Henry, day consultant), that the Alpha Hunter 9 rig had drilled a 27 inch hole to a depth of 328 feet (Kelly Bushing at 12 feet). John informed me that the rig had not drilled into a mine void as expected, but, had drilled through a coal seam from 192 feet to 199 feet. Alpha Hunter set 18.625 inch, 87.50lbs/ft casing to a depth of 317 feet. AEU elected to attempt to cement the casing string conventionally. Allied Oil and Gas Services pumped 72bbl fresh water, 25bbl of gel with flake, and 10bbl fresh water ahead of cement (capacity of pipe plus 10bbl). Allied pumped 923sk of Class A cement, 15.6 ppg, 1.2 yield, 2% CaCl2, and 0.25lbs/sk of flake. Circulation was established and 27bbl of cement returned to surface and stayed at surface while I was on site. The plug was bumped at 220psi and pressure was increased to 720 psi. Allied lost 50psi over 5 mins which is still within 10%. Floats did not hold, water was bled back into the pipe and shut in. AEU will wait 8 hours before drilling out the shoe.
1426078504 2015-03-10 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to check the status of the Nomac 83 rig that had moved onto location over the past week. At the time of my inspection I was informed by American Energy Utica consultant Mickey Brewer that the rig was drilling ahead at a depth of 2775 feet. Drilling had commenced on 3-9-15.
1427123394 2015-03-23 Drill / Deepen / Reopen No Violations On the date of inspection I am on site to check the status of the Side Track plug that was set on 3-20-15. I was informed by American Energy Utica Superintendent Van Dietrich that the Nomac 83 rig had missed their intended Kick Off Point and would not be able to drill the curve as planned. Van Dietrich informed me that they planned to set a Side Track Plug from the bottom of the hole at 9561 feet back into the previous casing approximately 100 feet. The previous casing was set at 8617 feet. I was informed by American Energy Utica consultant Lance Midgely that the Top of Cement was tagged at 8417. The new Kick Off Point was at 8788 feet. At the time of my inspection the Nomac 83 rig was drilling an 8.5 inch diameter hole at a depth of 10,831 feet into the lateral. For the plug back job, the rig ran 8061 feet of 5 inch diameter, 19.5lbs/ft drill pipe and 1500 feet of 2.875 inch tubing on bottom. O-Tex Pumping, LLC pumped 540sk of Class H cement, 17.5ppg, 0.94 yield, 0.4% C-35, 0.2% X-Air. O-Tex displaced with 120bbl of Oil Based Mud.
1471963189 2016-08-23 Production Wells No Violations
1480347257 2016-11-28 Production Wells No Violations
1541382137 2015-03-31 Drill / Deepen / Reopen No Violations On the date of inspection I met with American Energy Utica consultant Lance Midgely. Lance informed me that they had cemented the 5.5 inch production string the previous day. At the time of my inspection the rig was in the process of skidding to the next well which will be the 4H.
