0015052377 | 2015-03-03 | Administrative Inspection | No Violations | I received a call from Chesapeake consultant Gene Belesom regarding the conductor that was set the previous day. He informed me that he had tagged cement at 24 feet below the cellar and it took 1.75 cubic yards of ready mix cement to bring cement to surface. It will take additional cement to bring cement up into the cellar to create a seal. |

0150164813 | 2015-05-28 | Drill / Deepen / Reopen | No Violations | I was on location to check the drilling status of Nomac Rig #71. The rig is currently tripping in the hole with a new 8.75 bit and washing the pipe down to a depth of 5,498 to continue drilling the vertical section of the well. I reviewed the job logs for the surface and intermediate casing cement jobs and both jobs went as designed and there were no issues. The location is in good condition and all drilling equipment is on containment. No violations. |
0881071223 | 2016-11-23 | Administrative Inspection | No Violations | I called Mr. Ed Hashbarger at 10:46 AM in response to complaint number 5476 filed at 10:10 AM today. Mr. Hashbarger informed me that today at approximately 8:45 AM he was down feeding his animals, he heard his metal feed containers vibrating. He said the vibrating lasted for approximately 20 seconds, then within a minute the vibrating occurred again for approximately 10 seconds. According to Mr. Hashbarger, the event spooked his chickens and a goat. He did not feel any shaking or the vibration, just heard the feed containers vibrating. He was concerned that the event was caused by oil and gas operations in the area. The last stimulation on the closest well occurred in December of 2015. He also voiced concern about loud booms he and his wife had heard in the distance behind his home. Mr. Hashbarger has not leased his oil and gas rights to anyone and is concerned that his gas is being extracted without his knowledge or consent. The final survey of this permit number showed the wellbore to be on target and did not deviate out of the leased area. Mr. Hashbarger indicated that his attorney had advised him to file a formal complaint any time something occurred that might be related to oil and gas activities. This complaint was forwarded to David Rush the seismic manager at OhioNET Seismic. He responded via e-mail today at 11:03 AM that his department has received no seismic alerts for Jefferson County this morning. |

1348278272 | 2016-03-24 | Drill / Deepen / Reopen | No Violations | My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. The workover rig that was on site during last inspection on 3/12/2016 has been removed. All black milled liner has been removed from the site as have the portable frack tanks used during the frack plug drill-

1373993825 | 2018-01-18 | Preliminary Restoration | No Violations | On the day of my inspection I found this pad to contain one properly labeled producing well equipped with christmas tree style wellhead. Pad is fenced and gated. Production records are submitted through 3rd quarter 2017. Well is producing this date with 0 psi on surface casing gauge and 180 psi on intermediate casing gauge. Production equipment includes one gas production unit, one sand seperator, two 400 barrel steel brine tanks, and pipeline equipment. Site meet requirements of ORC 1509.072A for prelininary restoration. |

1431978618 | 2015-05-18 | Drill / Deepen / Reopen | No Violations | Location status check prior to Nomac Rig #71 moving onto location. The permit is posted at the lease road entrance and the gate is locked. The well pad is fenced, locked and bermed. The berms and stockpiled topsoil have been stabilized with perennial grasses. The tin horn (cellar), rat hole, and 20 conductor casing are covered by a steel grate and surrounded by tubular fencing. Secondary containment has been placed on the pad and the rig mats and primary containment rolls are stacked out and ready to be installed. Six (6) 500 barrel frac tanks have been spotted on a bermed containment area at the northwest corner of the drilling pad and there are three (3) skid steers on location. No violations. |

1457813820 | 2016-03-12 | Drill / Deepen / Reopen | No Violations | My inspection this day was in response to a phone call from a local citizen inquiring as to the activity he had observed on this site. Upon arrival at the site I found Mercer Drilling with a workover rig and crew pulling their work string of tubing from the well after having drilled out the frac plugs. Secondary containment was properly placed and maintained around the rig and work area. BOPs were in use on the well. Also in place on site were five 500 bbl. mobile flow back tanks which were also within secondary containment. The Chesapeake representative on site stated that their next step will be to run production tubing into the hole to approximately 9700 feet and then begin flow back operations so that the well can be put into production. The permit issued for this well is current and expires on 2/26/2017 |
