0459863700 | 2016-07-28 | Casing String Witnessed | No Violations | I am on site today accompanied by Mike Moreland to witness the running and cementing of 13.375 inch surface casing. Alpha Drilling drilled a 17.5 inch diameter hole to a depth of 1542 feet and set 1542 feet of 13.375 inch, 61 pound per foot casing. O-Tex pumped 1370 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. Approximately 82 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1,200 psi for 5 minutes. |
0700698778 | 2018-02-23 | Preliminary Restoration | No Violations | On the day of my inspection I found pad containing three properly labeled christmas tree type wellheads. All three wells are in production during my inspection. Outslopes of pad are wel vegetated, lease road is solid, and pad is gated/locked. Well has 1250 psi on flow line gauge, 2000 psi on tubing gauge, 1500 psi on production guage, 500 psi on intermediate gauge, and 200 psi on surface gauge. Production equipment includes three sand seperators, three GPUs, and two 500 barrel steel produced water tanks. Tanks are in metal containment. Production records are up to date in RBDMS. Site meets requirements of ORC 1509.072A for preliminary restoration. |
1476800321 | 2016-10-18 | Drill / Deepen / Reopen | No Violations | On the date of my inspection I am onsite for a status check. Patterson 292 with Ascent Resources has drilled to total depth for this permit and is cleaning up the hole in preparation for running the 5.5 inch production casing. During my inspection I found 2 small tears in an HDPE secondary containment. The company man was with me and indicated that he would repair the damage immediately. All possible contaminants are within proper containment. Pad drains remain in the closed position. I did not find any visible spills on the pad. |

1565082871 | 2016-07-26 | Drill / Deepen / Reopen | No Violations | I am onsite today to witness the hydrostatic test for the 18.625 inch diameter mine string. I witnessed drilling the plug. Alpha Drilling drilled to 277 feet, 20 feet beyond the last total depth of 257 feet. Fresh water was pumped into the 18.625 inch casing to surface and held static level for 30 minutes. |

1883699342 | 2016-07-25 | Casing String Witnessed | No Violations | I am on site today accompanied by Inspector Patrick Shreve to witness the running and cementing of 18.625 inch mine casing. Alpha Drilling drilled a 24 inch diameter hole to a depth of 257 feet and set 257 feet of 13.375 inch casing, mine void from 203 to 208 feet. O-Tex pumped 580 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. The wiper plug landed and pressure was increased to approximately 100 pounds per square inch (psi) over pump pressure, the casing held approximately 260 psi for 5 minutes. Approximately 24 barrels of good quality cement returned to surface. |
