2018-02-23 | Preliminary Restoration | No Violations | On the day of my inspection I found pad containing three properly labeled christmas tree type wellheads. All three wells are in production during my inspection. Outslopes of pad are wel vegetated, lease road is solid, and pad is gated/locked. Well has 1250 psi on flow line gauge, 1800 psi on tubing gauge, 1400 psi on production guage, 750 psi on intermediate gauge, and 50 psi on surface gauge. Production equipment includes three sand seperators, three GPUs, and two 500 barrel steel produced water tanks. Tanks are in metal containment. Production records are up to date in RBDMS. Site meets requirements of ORC 1509.072A for preliminary restoration.
2016-05-04 | Drill / Deepen / Reopen | No Violations | Express Drilling has one rig drilling two wells at the same time on this pad. They are drilling at 60 ft. today on both wells with a 26 inch auger bit. They are drilling this as part of a combined conductor, mine string, surface casing plan. They plan to drill down and should encounter a mine void at 140 ft. to 160 ft. Then drill an additional 30 to 50 ft. below the mine and set the combination casing. Drilling will not continue at night and the drilling will begin again thru the day only.

2016-07-24 | Casing String Witnessed | No Violations | I am on site today accompanied by Inspector Patrick Shreve to witness the running and cementing of 13.375 inch surface casing. Alpha Drilling drilled a 17.5 inch diameter hole to a depth of 1565.25 feet and set 1563 feet of 13.375 inch 61 pound per foot casing. O-Tex pumped 1370 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. Approximately 81 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1000 psi for 5 minutes.

2016-05-02 | Drill / Deepen / Reopen | No Violations | I am on site today accompanied by inspector Mark Moreland, were we witnessed Express Energy Services is in the process of setting the tin horn. At the time of my arrival Express Energy appeared to be having mechanical issues and was the auger rig was shut down. I will return at a later date to check on the progress of the drilling of the conductor mine string.
2016-05-08 | Casing String Witnessed | No Violations | Express Energy has reach total depth @ 100 in the 36 open hole section of this well bore. The 30 welded conductor pipe was ran to total depth. Express Energy was on site to cement the casing with 275 sacks of Class A cement. Cement returns to surface were established.
2016-07-22 | Drill / Deepen / Reopen | No Violations | I am onsite today to witness the hydrostatic test for the 18.625 inch diameter mine string. I witnessed drilling the plug. Alpha Drilling drilled to 270 feet, 10 feet beyond the last total depth of 260 feet. Fresh water was pumped into the 18.625 inch casing to surface and held static level for 30 minutes.
2016-07-08 | Casing String Witnessed | No Violations | I am on site today accompanied by Inspector Joe Zollars, to witness the running and cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 110 feet and ran 100 feet of 30 inch diameter conductor casing. Allied Oil & Gas Services LLC pumped 30 barrels of fresh water, then pumped 370 sacks of class A cement, 1.18 yield, 15.6 pound per gallon. Cement returned to approximately 2 feet below surface, topped off by Allied.
2016-07-06 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed the rig pads have been laid down and Ascent is in the process of setting the tin horn. Engineer Dakota Murdock stated that the conductor casing should be cemented on Friday morning.

2016-05-05 | Drill / Deepen / Reopen | No Violations | I arrived on site today to find nobody here, the gate is locked and no equipment is running. I contacted the Company Man Stan Cherry and he did not answer his phone. I left a message to inquire why they were not drilling. While I was making my report Express Drilling arrived and told me they had to pick up a gas detector in case they drilled into the mine void and it contained gas. They are at 85 ft. on both wells and should be ready to drill into the mine void on Friday. I contacted Region Supervisor about the permit conditions that were updated and sent out today. Brent Bear is to call Columbus for clarification. Brent did call me after his call to Columbus and advised that the casing plan has now reverted back to the original casing program. They will not be setting a combination string instead they will set conductor followed by a mine string.
2016-07-11 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witness Ascent is in the process of getting the top hole rigged up on location.
