0087179861 2016-02-29 Drill / Deepen / Reopen No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #3H partially drilled with only 20 inch conductor cemented in place. Tubular gates were in place around the cellar. Well identification number for this well was not found on the well but was on signage at entrance to access road. No drilling rig or any equipment was present on site.
0749495905 2016-12-08 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with a locked gate. The pad has 1 unidentified newly drilled well. Two 400 barrel tanks are being stored on the pad along with one frac tank and some miscellaneous containment pieces. The wellhead is equipped with a partial christmas tree valve and is surrounded by a metal fence.

1326211011 2016-08-05 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 80 drilled a 17.5 inch diameter hole to a depth of 970 feet and set 960 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 918 sacks of class A cement, 1.2 yield and 15.6 pound per gallon. Approximately 50 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1,015 psi for 5 minutes.
1455231032 2016-02-11 Casing String Witnessed No Violations On my inspection this day, Express Energy Services had drilled a 26 inch diameter hole to the depth of 120 feet, hitting water at 8 feet, and ran 120 feet of 20 inch 94#/ft conductor pipe. Express gained circulation and cemented pipe with 230 sacks Class A cement with 2% CaCl and .5#/sk. of floseal. Cement was displaced with 39.28 bbls of fresh water and had good cement returns of 8 bbls to surface.

1522078604 2018-03-26 Preliminary Restoration No Violations On the day of my inspection I found this pad containing one properly labeled well. Surface and intermediate annular pressures read 0 and 250 psi, respectively. Production equipment includes one sand seperator, one GPU, and two 400 barrel steel tanks, within their own steel containment. Production records are up to date. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration

Nomac Rig 80 drilled a 12.25 inch diameter hole to a depth of 2600 feet and ran 2600 feet of 9.625 inch 40 pound per foot casing. Schlumberger established circulation with fresh water, pumped 1,114 sacks of class A cement, 1.23 yield and 15.6 pound per gallon. Approximately 76 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 2,000 psi for 5 minutes.
