0898710598 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad containing one properly labeled producing wellhead. Surface annular PSI is 0 and intermediate is 300 psi. Production Equipment includes one gas production unit, one sand seperator, and two 400 barrel steel tanks, inside their own steel containment. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration.

1479132586 2016-11-14 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 74 drilled a 17.5 inch diameter hole to a depth of 829 feet and set 819 feet of 13.375 inch 54 pound per foot casing. O-Tex Pumping LLC established circulation with fresh water, pumped 690 sacks of class A cement, 1.20 yield and 15.6 pound per gallon. Approximately 34 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 930 psi for 5 minutes.

1811614510 2016-11-16 Drill / Deepen / Reopen No Violations I am onsite today to inspect the well pad during drilling operations. Nomac 74 is currently drilling ahead at approximately 5000 feet. All hazardous materials are being stored within secondary containment. All secondary containment appears to be in good condition, free of obvious punctures or rips.
