0080009066 2016-04-14 Drill / Deepen / Reopen No Violations I am on site today accompanied by Inspector Mark Moreland, to meet with the company man Brett Farnsworth for information about the conductor cement job that will occur promptly. Brett informed us that the cement job was scheduled for 8:30 pm that night. While on site I witnessed Express Energy Services was drilling a 26 inch diameter hole with an auger rig. During the conduction of my inspection I performed a site overview, were I found no signs of pollution or contamination. I will be back on site tonight to witness the cementing of the conductor job.

0453533162 2016-04-15 Drill / Deepen / Reopen No Violations I am on site today accompanied by Inspector Mark Moreland, to witness the cementing of the conductor casing. Express Energy Services drilled a 26 inch diameter hole to a depth of 100 feet and ran 100 feet of 20 inch diameter, 94 pound per foot casing. O-Tex Cementing Service is the cement contractor on site. O-Tex displaced with 32 bbl. of fresh water and then pumped 245 sacks of Class A 1.18 percent yield/15.6 pound cement. I witnessed cement returns to surface and 2 feet of cement was left in the tin horn, sealing off the tin horn and conductor.
0584368683 2016-06-30 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Nomac Rig No. 74 in the process of drilling the lateral section of the well bore. The company man stated that total depth would be reached within the next two days. While on site I performed a walk around the location and found no signs of pollution or contamination.
1007863809 2016-03-24 Surface Facility Construction No Violations On my inspection today, I found construction on this well pad nearly complete. The pad itself was complete at final grade and final gravel and rolling has been applied. Well pad slopes had not yet been seeded. All erosion and sedimentation controls are in place. Signage has been placed at the access road entry point with required well identification information.

1133436737 2016-11-21 Preliminary Restoration No Violations I am onsite today to perform a preliminary restoration inspection. All drilling equipment has been removed from the pad. The well has been placed into production. There are currently 2 frac tanks being stored onsite.
1239456372 2016-09-27 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained lease road with a fence and gate. The pad has 1 drilled well. The pad is surrounded by healthy vegetation. The wellhead is equipped with a christmas tree valve. There are no pressure gauges tied into the wellhead. Frac equipment is currently being removed from the site. Production equipment that is onsite but not assembled yet includes a sand separator, glycol dehydration unit, gas production unit, and one 400 barrel tank within a metal dike.

Company Man David Newman stated the rig would be drilling the horizontal section within the next couple of days and completing the well by the end of the week.
1467383323 2016-07-01 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 84 is drilling ahead on the lateral section of the well using oil base mud at 16,500 ft. They plan to finish drilling tonight and will run a series of logs following the drilling operation. The series of logs will be thru the bit type logs done before running the production casing. The rig plans on moving to the Shultz location after completing the current well.
