0098267619 2019-04-23 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain three properly labeled, producing wells. This well is producing through the 5.5 inch diameter casing. This well is producing from each side of the well head. Production equipment includes five sand seperators, five GPUs, and two 400 barrel steel tanks, in their own containment. Five desanders remain on location as well as two agricultural tractors, three water trucks, one lime truck, and three frac tanks. Area around the pad, (outside of pad disturbance limit) has been plowed. There is also a gravel road for pipeline right of way access that appears of recent construction. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 750 psi. Pad slopes are stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
0118330106 2019-01-08 Completion Testing No Violations On the day of my inspection I witnessed the BJ frac crew rigging down equipment and moving off of location. I spoke with company man Dave and he said they are moving to the cevik pad in columbiana county. Kill plugs were set in this well this morning at 8860 and 8806 feet. Surface annular pressure is zero and intermediate annular pressure is 50.
0407444185 2018-08-21 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 571 rig moving in to drill the 1H and 3H wells. ID to Company man Joe. The 5H well has had SIMOPS procedures completed, see my report for that well on this date for more information. The rig is not completely on location yet and they are hoping to spud late in the weekend.
0809254817 2019-01-22 Completion Testing No Violations On the day of my inspection I witnessed Stone Rig 15 was moving off of location. This pad has had the frac plugs drilled. Several condtractors were on location working on surface production equipment and plumbing. Surface annular pressure is 0, and intermediate annular pressure is 300.

1530282063 2018-06-29 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 36 inch diameter hole to 120 feet. 120 feet of 30 inch diameter conductor casing was run. O-tex/CJs was onsite with 345 sacks of class A cement. Additives include 1% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 16.6 ppg and yield of 1.05. 30% percent excess ran. Cement was displaced with 92 barrels of fresh water. 4 barrels of cement returned to surface.
1546454794 2019-01-02 Completion Testing No Violations On the day of my inspection I witnessed BJs frac crew onsite tied in to both of these wells. Stage 75/88 has plug set at 11513, with bottom perfs at 11498 and top perfs at 11382. This stage is next for stimulating. They are zipper fracking between the 1 and 3 wells. Surface annular pressure is 0 and intermediate annular is 82.
1673360151 2018-12-12 Completion Testing No Violations On the day of my inspection I witnessed a BJ Energy frac crew on location. Identified myself to Company man Terry Hickman. They are currently fracking stage 19/88. Plug set at 19918, with bottom perforation at 19902 and top perforation at 19782. Perforations made with 5 guns, 6 shots each. Surface and intermediate annular pressures were 0.
1884421552 2018-08-27 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 628 feet. Previous casing set and cemented was 20 inch diameter surface casing to a depth of 507 feet. Costys ran 622 feet of 13.375 inch diameter casing, p-110, 54.5 lbs/foot. Shoe track is 49 feet long. DV tool set at 515 feet. Packer set at 513 feet. Schlumberger pumped 32 barrels of class A surfaceset cement on first stage and 114 barrels on second stage. Cement weighed with mudscale at 15.6 ppg and yield of 1.21. 50% excess ran in open hole. 10 barrels of cement returned to surface on first stage and 20 barrels of cement returned on second stage. Floats held with 1 barrel back to truck.

Costys ran 507 feet of 20 inch diameter casing, j-55, 94 lbs/foot. Shoe track is 45 feet long. Schlumberger was onsite with 735 sacks of class A surfaceset cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.21. 30% excess ran in open hole. Cement displaced with 164 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 34 barrels of cement returned to surface weighing 14.3, and floats held with .5 barrels back.
