0098097779 2016-07-11 Casing String Witnessed No Violations I am on site today accompanied by Inspector Joe Zollars to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 74 drilled a 17.5 inch diameter hole to a depth of 636 feet and set 621 feet of 13.375 inch casing, float collar is set at 618 feet. Schlumberger established circulation with fresh water, pumped 639 sacks of class A cement, 1.23 yield and 15.6 pound per gallon. Approximately 113 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 850 psi for 5 minutes.
0520213836 2019-01-22 Completion Testing No Violations On the day of my inspection I witnessed Stone Rig 15 was moving off of location. This pad has had the frac plugs drilled. Several condtractors were on location working on surface production equipment and plumbing. Surface annular pressure is 0, and intermediate annular pressure is 0.
0522289567 2018-09-18 Drill / Deepen / Reopen No Violations On the day of my inspection I was accomapanied by Inspector Ken Fry. We identified ourselves to company man Neil Brown. Patterson 571 is currently drilling their 8.75 inch diameter hole at 5000 feet on the 3h well. This is a SIMOPs pad. Annular pressures on this temporarily abandoned well are zero psi. Ken and I also reviewed the anticollison reports for the wells.
0781040527 2016-07-02 Casing String Witnessed No Violations I am on site today accompanied by inspector Joe Zollars, to witness the running and cementing of 30 inch conductor casing. Express Energy Services drilled a 36 inch diameter hole to a depth of 130 feet and ran 120 feet of 30 inch diameter conductor casing. Express Energy Services established circulation with fresh water, then pumped 330 sacks of class A cement, 1.18 yield, 15.6 pound gallon. I witnessed good cement returns to surface, cement maintained a static level.

The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 780 psi for 5 minutes.

1296033662 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed Patterson 571 rig moving in to drill the 1H and 3H wells. ID to Company man Joe. According to Chesapeake the production tubing has been removed and will be sent for inspection. A retrieveable bridge plug was set at 7310 feet. Casing bled down to zero, and had a good negative test. Loaded hole with brine and pressured up to 3200 psi for 10 minutes. Good negative and positive tests on the bottom plug. A retrieveable bridge plugset at 7140 feet. Top casing off with brine and pressure up to 3100 psifor 10 minutes. Good positive test on the top plug. Casing is loaded with brine to surface. Tubing head nippled down to below grade and nippled upa night cap. Flowlines and Production equipment are depressurized and blown down. Production equipment includes five GPUs, 5 sand seperators, and two 400 barrel steel tanks in their own containment.
1847293386 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain one properly labeled producing well. Surface and intermediate annular pressures read 0 and 300 psi, respectively. Production equipment includes one sand seperator, one GPU, and one 400 barrel steel tank, within its own metal containment. Site is stable and vegetated and meets requirements of ORC 1509.072A for preliminary restoration.
1850660029 2018-09-18 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0636. Jefferson County Inspector Dylan Pendleton was present during the inspection. During the inspection, Dylan and I met with company representative Neil Brown. At the time of my inspection well #2-0636 was temporarily abandoned to meet SIMOPS permit conditions for offset wells on the pad. The well head annular valves were plumbed above grade and were open for monitoring. The well head had been lowered to the top of the B section and a night cap installed. Steel livestock gates were placed around the well head. The cellar was filled with water. No sheen or gas bubbles were observed. Patterson drilling rig #571 was on location and drilling offset well #2-0634. The proposed and approved SIMOPS conditions were met.
2040854246 2019-04-23 Production Wells No Violations On the day of my inspection I witnessed this pad to contain three properly labeled, producing wells. This well is producing through tubing on natural reservoir pressure. Production equipment includes five sand seperators, five GPUs, and two 400 barrel steel tanks, in their own containment. Five desanders remain on location as well as two agricultural tractors, three water trucks, one lime truck, and three frac tanks. Area around the pad, (outside of pad disturbance limit) has been plowed. There is also a gravel road for pipeline right of way access that appears of recent construction. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 0 psi. Pad slopes are stable and vegetated.
