0328056083 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad containing three properly labeled producing wellheads. Surface annular PSI is 0 and intermediate is 100 psi. Production Equipment includes three gas production units, three sand seperators, and three 400 barrel steel tanks, inside their own steel containment. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration.
1470749689 2016-08-09 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 24 inch diameter hole to a depth of 122 feet and ran 120 feet of 20 inch diameter 94 pound per foot conductor casing. Express Energy Services pumped 16 barrels fresh water as a pre-flush, then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.5 pound per sack flow seal. 50 percent excess was pumped and there was good circulation. I witnessed approximately 18 barrels of good cement return to surface, cement maintained a static level. There were no centralizers or flow shoe ran.
1483711839 2017-01-06 Completion Testing No Violations On the date of my inspection I found Chesapeake setting up equipment to begin hydraulic fracturing on all three wells on this pad. Frac stacks have been rigged up on the wells. Halliburton is currently setting up their sand containers on site. Containment for other equipment is also being set up. Chesapeake estimates frac operations to begin on 1/9/17.
1485438845 2017-01-26 Administrative Inspection No Violations Upon receiving an email from Mike Moreland District Supervisor, on complaint # 5517, I phoned Darla MCConnell. She voiced concerns about Chesapeake activities on the Henry Pad. Darla stated that the lease road is approximately 100 feet from there house, and 75 feet from there barn. Her concerns are about the noise, the noise from all the truck traffic, the dust in the summer, and a concern about how the truck stage before a job. At the present time they are fracing the well. They are to meet with a person from Chesapeake at 9:00 am this morning. I will visit the location.
1489066660 2017-03-09 Completion Testing No Violations On the date of my inspection I found a well maintained lease road with a fence and gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with an open gate. The pad has 3 wells, 3 sand separators, 2 glycol dehydration units, 3 gas production units, and 3 shared 400 barrel tanks within a metal dike. The wellhead is equipped with a christmas tree valve. The wells have recently been stimulated and construction crews are onsite installing the production equipment and piping from the wellheads.
1515956907 2016-09-29 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 80 drilled a 17.5 inch diameter hole to a depth of 640 feet and set 627 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 570 sacks of class A cement, 1.21 yield and 15.6 pound per gallon. Approximately 25 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 1000 pounds per square inch (psi) over pump pressure, the casing held approximately 1200 psi for 5 minutes.
