0091445085 2016-10-12 Drill / Deepen / Reopen No Violations Today I am onsite to witness topping out the conductor for this well. Cement was measured at 63 feet below ground surface with water at 58 feet. Grout was used to top out the conductor annulus since there was only 5 feet of water on top of the cement. The grout was mixed to an acceptable consistency and minimal water returned to surface. Grout was brought to 80 inches in the tin horn. The seal in the bottom of the tin horn was approximately 18 inches thick.
0474858894 2016-11-22 Drill / Deepen / Reopen No Violations On the date of my inspection I found Nomac 80 rigging up on the pad. The rig is approximately 20 percent moved in. I spoke with Chesapeake company man Chris, who indicated that the rig should be completely moved by early morning on 11/23.

A Profrac crew is onsite to stimulate the 1H, 5H, and 205H wells. The 9.625x5.5 annular pressure is 56 psi. Production gauges reads zero.
0686514310 2017-04-06 Completion Testing No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad has 2 drilled wells, 2 sand separators, 2 gas production units, and 2 shared 400 barrel tank within a metal dike. The pad is surrounded by an earthen dike covered by healthy vegetation. Both wells were drilled in 2016. Completion equipment has started to be moved onsite.

1374822042 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad containing two properly labeled producing wellheads. Surface annular PSI is 0 and intermediate is 580 psi. The intermediate annular pipe on this well and the 661 well have a motor valve on them and both of these wells intermediate annuli are plumbed together into a buried flowline. Production Equipment includes two gas production units, two sand seperators, and two 400 barrel steel tanks, inside their own steel containment. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration.
1476282956 2016-10-11 Drill / Deepen / Reopen No Violations 0930 am - Chesapeake set the conductor for this permit number on 10/10/16. David Newman informed me this morning that cement was tagged at 96 feet down the annulus, with water at 68 feet. He planned to ready-mix the remaining portion to surface. I was onsite to verify these numbers. I spoke with Mike Moreland about this plan and expressed that I thought a 1 inch tremie pipe and grout should be used to bring cement to surface and he agreed. I informed David of the requirements for completing the cement job. This work will be completed later today.
1480121458 2016-11-25 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 80 drilled a 17.5 inch diameter hole to a depth of 835 feet and set 829 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 748 sacks of class A cement, 1.21 yield and 15.6 pound per gallon. Approximately 41 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 800 psi for 5 minutes.
