0066727537 2018-06-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 was drilling an 8.75 inch diameter hole at 6335 feet. Walk around inspection revealed no issues with containment. No flare during inspection.
0217725251 2018-05-15 Casing String Witnessed No Violations Pad address is 1136 township road 259, Richmond Ohio. On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 230 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50 percent excess ran. Cement was displaced with 39 barrels of fresh water. No returns to surface. Cement fell back from ~30 to 60 feet after puming. Company man David Newman will tag cement after it solidifies and call me with results. Top out will be 5/15 or 5/16.

0694469487 2018-07-13 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed wyonming casing running 5.5 inch diameter production casing. 23#/ft grade p-110. Well was drilled to a total depth of 21635. Kick off point was 8338 and landing point was 9329. this is the last well on this pad and rig will leave for Pennsylvania starting Monday. Cement job will be late tonight.
0977684844 2018-08-14 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew rigged up on well. ID to company men Fletcher Brand and Rudy. Surface annular pressure is zero. Intermediate annular pressure is 289 psi. A composite plug is at 10560 and operator has a tractor with a mill on it coming out to try and mill the plug out. They fracked stage 1 and were setting plug for casing test. Plug failed and needs milled out.
1044405085 2018-08-20 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on site. ID to company men Rudy and Fletcher. Surface annular pressure is 0 and intermediate annular pressure is 216 psi, prior to frac. Last week they had brought a tractor on location that had a mill. They went down hole and milled out the center of the plug but did not get the slips. They anticipate bringing in Stone rig 15 or coiled tubing unit later in week to drill out the plug so they can continue fracking.
1397885549 2019-04-23 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain 3 producing wells. Production equipment includes three sand seperators, three GPUs, and six line heaters that are not plumbed in. Three 400 barrel steel produced water tanks are in their own containment. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 1400 psi. The 9.625x5.5 annular is plumbed into a flowline. Pad slopes are stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
1526474304 2018-05-16 Casing String Witnessed No Violations On the day of my inspection I witnessed Superintenant David Newman tag cement at 68 feet from ground surface. Backside was grouted back to surface with approximately 8 yards of grout.
1530542055 2018-07-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Schlumberger pump an 800 foot plug with 25% excess (~1000 feet total). Bottom of plug is 6795 feet and top is estimated around 5800 feet. 28 barrels of 13.0 ppg mud push was pumped prior to 74.4 barrels of 17.5 ppg class H cement with .94 yield. Cement was followed with 12 barrels of 13 ppg mud push. Plug displaced with 102 barrels of oil based mud. This kick off plug was set because the rig lost a fish in the wellbore. Total depth drilled was 8340. Top of fish is at 6847 feet, fish is 1445 feet long and consists of 3 joints of drill pipe, 5 crossover subs, 2 antenna subs, 2 indexing tools, drill collars, MWD tool, shock sub, mud motor, and bit.
