0679856063 2018-08-14 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac on pad. ID to company men Fletcher Brand and Rudy. 9 wireline was coming out of this well. They just perforated stage 7. Stage 7 has plug set at 20756, bottom perforation at 20736, and top perforation at 20577. Perforations were shot with 5 guns with 6 shots each on 40 foot spacing. Surface annular pressure is zero. Intermediate annular pressure is 47 psi.
0847633996 2018-08-20 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on site. ID to company men Rudy and Fletcher. Surface annular pressure is 0 and intermediate annular pressure is 8 psi, prior to frac. Nine wireline set stage 22 frac plug at 17779 feet. Bottom perforation is at 17760 and top perforation is at 17600. Well perforated with 5 guns with 6 shots on 40 foot spacing. Currently fracking the 3H during my inspection
1034969361 2018-07-27 Completion Testing No Violations On the day of my inspection I witnessed 9 energy running a CBL on this well. No frack crew onsite yet. Company man said they would be opening the toe valves on Monday. Surface and annular gauges read 0.
1496203976 2018-05-15 Casing String Witnessed No Violations Pad address is 1136 township road 259, Richmond Ohio. On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. The first 30 feet was augered 30 inches wide. Hole was full of water and only a couple feet below cellar floor prior to starting job. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 350 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50 percent excess ran. Cement was displaced with 39 barrels of fresh water. 25 barrels of cement returned to surface.
1540392816 2018-06-04 Drill / Deepen / Reopen No Violations On the day of my inspection I found Patterson 580 drilling an 8.5 inch diameter hole in the lateral. Mud weigh is 11.3 ppg and an intermittent lazy flare was burning during my inspection. Rig is at 18312 feet. Kick off point was 8725 and curve was landed at 9765. Planned total depth is 21992 feet. Trucks unloading production casing during my inspection. Walk around rig revealed containment is functioning.
1794522328 2019-04-23 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain 3 producing wells. Production equipment includes three sand seperators, three GPUs, and six line heaters that are not plumbed in. Three 400 barrel steel produced water tanks are in their own containment. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 500 psi. Pad slopes are stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
