0099795449 2019-04-23 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain 3 producing wells. Production equipment includes three sand seperators, three GPUs, and six line heaters that are not plumbed in. Three 400 barrel steel produced water tanks are in their own containment. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 500 psi. Pad slopes are stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.

Stage 6 has plug set at 20747, bottom perforation at 20727, and top perforation at 20568. Perforations were shot with 5 guns with 6 shots each. 3000 gallons of 15% HCl pumped along with 100000 lbs of 100 mesh sand and 230000 lbs of 40/70 mesh sand. Total job volume pumped was 5452 barrels. ISIP was 6219. Shut in pressure on well was 5876 at about 3 minutes. They do not do a 5, 10, or 15 minute ISIP. Average pressure was 9846 and average rate was 85 barrels per minute. Break down pressure was 7143. Surface annular pressure is zero. Intermediate annular pressure is 74 psi.
0659539647 2018-05-16 Casing String Witnessed No Violations Pad address is 1136 township road 259, Richmond Ohio. On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 230 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50 percent excess ran. Cement was displaced with 39 barrels of fresh water. 15 barrels of cement returned to surface.

1297010489 2018-05-17 Casing String Witnessed No Violations On the day of my inspetion I witnessed a worm had developed in the cement that was pumped yesterday. The information I received this morning via phone call indicated cement had fallen to 50. Upon inspection of the cement I found a worm hole that was 8 feet deep. Kris Gonzales, Chesapeake Superintenant was also onsite because of the misinformation. I stuck around to watch Express do mouse holes while waiting on grout truck to fill the hole. One yard placed in worm hole and cellar floor.
1528833251 2018-06-12 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 was drilling an 8.75 inch diameter hole at a depth of 6100 feet. Walk around location revealed no issues with containment. Company Man Danny Lambert was advised to get a vac on location to dispose of rain water within containment as it was nearly full near the northwest corner of the pad. Area has received significant rains over the last few days and more rain is expected.
1529691482 2018-06-22 Casing String Witnessed No Violations On the day of my inspection I witnessed Wyoming casing ran 21786 feet of 23#/ft 5.5 inch diameter production casing with a stick 3.7 foot stick up. Shoe track is 88 feet long. O-tex/CJs onsite with 351 barrels (1480 sacks) of 14 ppg lead cement and 681 barrels (3160 sacks) of 15.6 ppg tail cement. Yields are 1.33 and 1.21 respectively. Lead cement is 50% class H/ 50% poz mix. Tail cement is class H. 100 barrel of 13 ppg spacer. Job is figuered with 15% excess on 8.75 inch diameter hole. Casing was centralized. 458 barrels of displacement calculated.
