0426708599 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 1000 psi intermediate, 0 psi tubing, and 4000 psi production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
0594462914 2017-12-12 Production Wells No Violations On the date of my inspection I found a well maintained lease road with an unlocked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with an unlocked gate. The pad has 3 wells, 4 sand separators, 3 gas production units, and two 500 barrel tanks within a metal dike. Only the sand separator for this permit has an identification tag on it. Since the time of my last inspection, Ascent Resources has drilled 2 more wells. This permit number is currently shut in and the pressure gauges have been removed. There is currently a metal cage around the wellhead. ESS Corporation is currently onsite with SEI working on the new production equipment. There is also some miscellaneous equipment on the pad related to the equipment installation activities. The Ascent company man indicated that he thought frac operations would begin on the 2 new wells within 3 weeks. The 2H will remain shut in until they flow back the 4H and 6H.
1471527905 2016-08-17 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 13.375 inch surface casing. Alpha Drilling drilled a 17.5 inch diameter hole to a depth of 1,575 feet and set 1,550 feet of 13.375 inch, 61 pound per foot casing. O-Tex established circulation then pumped 1,320 sacks of class H cement, 1.2 yield and 15.6 pound per gallon. Approximately 43 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1,450 psi for 5 minutes.
1478617727 2016-11-08 Drill / Deepen / Reopen No Violations I was notified of a small spill on the township road adjacent to the access road for this well pad. The spill occurred at approximately 4 pm on 11/8/16. Warren trucking was moving the Patterson 284 rig to another pad for Ascent Resources when some oil based drilling fluid leaked onto the roadway. The spill quantity reported was approximated at 5 gallons. The spill covers an area approximately 30 feet long by 5-8 inches wide and widens out to approximately 5 feet where the truck stopped to fix the leak. The roadway is stained with minor residue. The spill did not leave the roadway but is approximately 30 feet from Long Run waterway. I found the location of the spill to be at the following coordinates: 40.18391, -80.86325.

2125237803 2016-08-15 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Alpha Drilling drilled a 24 inch diameter hole to a depth of 400 feet and set 400 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 820 sacks of class H cement, 1.2 yield, 15.6 pound per gallon, 1.5 percent Calcium Chloride, 0.25 pounds per sack of cello flake. No cement returned to surface. 5 barrels of cement was pumped through approximately 100 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket at approximately 175 feet, after setting for 2 hours, cement was pumped to surface and remained static.
