0838932067 2018-03-22 Preliminary Restoration No Violations On the day of my inspection I found this pad containing two properly labeled producing wellheads. Surface annular PSI is 0 and intermediate is 520 psi. The intermediate annular pipe on this well and the 644 well have a motor valve on them and both of these wells intermediate annuli are plumbed together into a buried flowline. Production Equipment includes two gas production units, two sand seperators, and two 400 barrel steel tanks, inside their own steel containment. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration.

1416239980 2017-04-06 Completion Testing No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad has 2 drilled wells, 2 sand separators, 2 gas production units, and 2 shared 400 barrel tank within a metal dike. The pad is surrounded by an earthen dike covered by healthy vegetation. Both wells were drilled in 2016. Completion equipment has started to be moved onsite. Jody Jones with Chesapeake called to inform me that when they began to install the wellhead on this well, the intermediate casing has approximately 1600 pounds per square inch of pressure. He indicated that this pressure would build back up over 2 days if it was blown down. They finger printed the gas and believe it is coming from the Lockport Formation based on other wells nearby. Chesapeake is working a plan to mitigate this issue.
1476450471 2016-10-14 Drill / Deepen / Reopen No Violations I am on site today to verify the conductor top-out job from 10/13/16. Express poured grout down the backside of the 20 inch conductor and filled the tin horn to approximately 80 inches below ground surface. The grout seal in the tin horn is approximately 16 inches thick.
1481338571 2016-12-09 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Rig 80 drilled a 17.5 inch diameter hole to a depth of 760 feet and set 750 feet of 13.375 inch 54 pound per foot casing. Schlumberger established circulation with fresh water, pumped 640 sacks of class A cement, 1.21 yield and 15.6 pound per gallon. Approximately 37 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 800 psi for 5 minutes.
1481638879 2016-12-13 Drill / Deepen / Reopen No Violations Today I am onsite to perform a walk around during drilling operations. Nomac 80 is currently drilling ahead at 6693 feet. They are expecting to reach their kickoff point for the curve tonight. During my site walk around, I found several holes in the secondary containment surrounding the drilling rig and associate equipment. I spoke with Chesapeake company man Jackie Holik about the containment issues and he indicate that he would call in for repairs today. No other issues were found.
1482244129 2016-12-20 Drill / Deepen / Reopen No Violations On the date of my inspection I found Nomac 80 had reached a total depth of 19,252 feet and had run 19,249 feet of production casing. Schlumberger is onsite and rigged up to cement the production casing. They are currently waiting on a cement truck to start the job. I verified that Chesapeake had made the repairs to their containment that I found on 12/13/16.
1543760218 2016-10-12 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 26 inch diameter hole to a depth of 120 feet and ran 120 feet of 20 inch diameter 94 pound per foot conductor casing. Express Energy Services pumped 3 barrels fresh water as a pre-flush, then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. 50 percent excess was pumped and there were no returns to surface. Cement was measured at approximately 43 feet below ground level. Express will top out with grout tomorrow morning.

A Profrac crew is onsite to stimulate the 1H, 5H, and 205H wells. The 9.625x5.5 annular pressure is 1180 psi. Production gauges reads zero.
