0836745437 2017-11-16 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 drilled wells, 3 vertical sand separators, 3 gas production units, and three 400 barrel tanks within a metal dike. There are not any identification tags on the production equipment. All of the production equipment is surrounded by concrete barriers. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. Currently Stone Well Service has a rig on site for drill out operations. They have 8 plugs left to drill out on this well. It is the last well on the pad.
1492267060 2017-04-15 Casing String Witnessed No Violations Upon arrival on location I found the crew ready to cement the conductor casing. 120 feet of 20-inch casing was run and cemented back to surface. The cementing was done by Express Energy. 43.34 bbl. of class A with 2% calcium and 1 /2 pound of flake per sack was pumped and displaced with 39.7 bbl. of freshwater. Established circulation and circulated 17 bbl. of good cement returns to surface.
1499384766 2017-07-06 Casing String Witnessed No Violations I am on site to witness the cementing of 13.375 inch surface casing. Nomac 80 drilled a 17.5 inch diameter hole to a depth of 626 feet and set 616 feet of centralized 13.375 inch 54.5 pound per foot casing. Schlumberger established circulation with fresh water, pumped 25 barrels of bentonite gel spacer followed by 10 barrels of water, then pumped 520 sacks of class A cement (1.2 yield and 15.6 pound per gallon, Surface Set). The wiper plug was released and Schlumberger displaced with 88 barrels of fresh water. By the time the plug landed approximately 29 barrels of cement had returned to surface. When the plug landed, pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 900 psi for 5 minutes.
1502721377 2017-08-14 Casing String Witnessed No Violations On the date of my inspection I found Patterson Drilling rig 580 displacing the cement for the production casing. Patterson drilled an 8.5 inch diameter hole to a depth of 17999 feet and set 17999 feet of 5.5 inch 20 pound per foot casing. Schlumberger broke circulation and pumped 100 barrels of cemprime scrub ahead then pumped 4165 sacks of class H cement, 1.19 yield and 15.6 pound per gallon. Approximately 30 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 4800 psi for 5 minutes.

Halliburton moving equipment onto the pad for hydraulic fracturing operations. The Halliburton foreman expects to be moved in and beginning operations by 9/23/17. The production equipment has already been installed on the pad and currently includes 3 sand traps, 3 gas production units, and three 400 barrel tanks surrounded by a metal dike. The pad is surrounded by an earthen berm covered by healthy vegetation with the exception of where the pipeline has recently tied into the pad.
2059773011 2017-10-12 Completion Testing No Violations On the date of my inspection, I found a well maintained lease road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered with healthy vegetation. All three wells on the pad have recently been hydraulically fractured. Equipment remaining on site from frac operations include a man lift, 3 residual waste containers, 13 frac tanks, a skid steer, a 3 forklifts, 2 roll off trash containers, walls components for an above ground storage tank, and the secondary containment mats. The wells are equipped with a wellhead that has chains looped through the valve handles to prevent tampering. Production equipment on the pad includes three 400 barrel tanks within a metal dike, 3 sand separators, and 3 gas production units. Currently a pipeline crew is tying the gas pipeline into the production equipment.
