0376942330 2017-07-03 Drill / Deepen / Reopen No Violations On the date of my inspection I found Nomac drilling rig 80 moving onto the pad and rigging up on this permit. According to the Chesapeake company man, they intend to start drilling the surface string on 7/4. He indicated that they would be drilling all of the surface strings for the wells on this pad before going back to 1H and drilling it to completion. The pad is surrounded by a fence and berm with healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
0601852776 2017-10-12 Completion Testing No Violations On the date of my inspection, I found a well maintained lease road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered with healthy vegetation. All three wells on the pad have recently been hydraulically fractured. Equipment remaining on site from frac operations include a man lift, 3 residual waste containers, 13 frac tanks, a skid steer, a 3 forklifts, 2 roll off trash containers, walls components for an above ground storage tank, and the secondary containment mats. The wells are equipped with a wellhead that has chains looped through the valve handles to prevent tampering. Production equipment on the pad includes three 400 barrel tanks within a metal dike, 3 sand separators, and 3 gas production units. Currently a pipeline crew is tying the gas pipeline into the production equipment.
1115581637 2017-09-19 Completion Testing No Violations On the date of my inspections I found a well maintained access road with an unlocked gate. The pad has 3 drilled wells. Halliburton moving equipment onto the pad for hydraulic fracturing operations. The Halliburton foreman expects to be moved in and beginning operations by 9/23/17. The production equipment has already been installed on the pad and currently includes 3 sand traps, 3 gas production units, and three 400 barrel tanks surrounded by a metal dike. The pad is surrounded by an earthen berm covered by healthy vegetation with the exception of where the pipeline has recently tied into the pad.
1490627894 2017-03-23 Administrative Inspection No Violations Response to complaint #5562. Theresa Hendricks called at 8:56 AM on 3/23 to follow up on a complaint she had filed at 3:35 PM on 3/22. She informed me that she was having trouble with the contractor constructing the Acton well pad. She indicated that a construction truck had run off the road into a drainage area for a spring near her driveway on Township Road 371. The damage caused by the truck caused the water to leave the channel and flow down the road and was flooding her driveway. She indicated that the altered flow was also impacting a wetland nearby. She was frustrated by the lack of response from the township and county agencies she had contacted about the issue which began in February. After speaking with Supervisor Mike Moreland about the complaint, he indicated that the issues were outside of our jurisdiction and that the matter would have to be taken up with the township. I called Ms. Hendricks at 7:44 AM on 3/24 to inform her that unfortunately I could not do anything about issues at hand, and that she would have to continue to contact the township.
1500393735 2017-07-18 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 580 tripping out of the hole to run their 5.5 inch production casing. They reached their total depth for this permit number of 19115 feet. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. I found a small tear in containment near the catwalk, and multiple punctures in the containment surrounding a portable fuel tank. There was a light generator and 2 pallets of chemicals outside of containment. I notified the Chesapeake company man of the issues who indicated they would take care of them today. The pad drain valves are in the closed position and do not appear to be leaking.
1770566393 2017-07-10 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 580 drilling the production string of this well. They are currently drilling ahead at approximately 6263 feet. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. I found a small area behind the chemical storage where some calcium chloride had spilled onto the berm and has killed some vegetation. I also found the area behind the cuttings neutralization and stabilization box to be covered with overflow from the box. I notified the Chesapeake company man of both of the issues who indicated they would clean it up as soon as possible. The pad drain valves are in the closed position and do not appear to be leaking.
1789529304 2017-11-16 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 drilled wells, 3 vertical sand separators, 3 gas production units, and three 400 barrel tanks within a metal dike. There are not any identification tags on the production equipment. All of the production equipment is surrounded by concrete barriers. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. Currently Stone Well Service has a rig on site for drill out operations. The plugs in this well have been drilled out and they are currently working on the 9H.
