0255631751 2017-08-21 Completion Testing No Violations On the date of my inspection I found a work over rig from Stone Well Service rigged up on permit 34-081-2-0675 attempting to re-establish the ability to pump down hole by drilling out 2 stage plugs. Halliburton is also on site with a frac crew waiting for the work over rig to perform the maintenance on the wells so they can continue with frac operations. All of the wells on the pad require some maintenance before frac operations can resume. This well has been hydraulically fractured through stage 11 of 46 stages. Ascent anticipates the frac operations to be able to resume by 8/24 or 8/25.
0388687932 2017-07-18 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 571 drilling their production string at 15167 feet. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. I found several punctures in the containment near the catwalk, a puncture in the containment near the drilling fluid tanks. The puncture near the drilling fluid tanks is actively leaking. I notified the Chesapeake company man of the issues who indicated they would take care of them as soon as possible. The pad drain valves are in the closed position and do not appear to be leaking.
1499697624 2017-07-10 Casing String Witnessed No Violations I am on site to witness the cementing of 9.625 inch surface casing. Patterson 571 drilled a 12.25 inch diameter hole to a depth of 2016 feet and set 2008 feet of centralized 9.625 inch 40 pound per foot casing. Schlumberger established circulation with fresh water, pumped 25 barrels of bentonite gel spacer followed by 10 barrels of water, then pumped 736 sacks of class A cement (1.2 yield and 15.6 pound per gallon, Surface Set). The wiper plug was released and Schlumberger displaced with 149 barrels of fresh water. By the time the plug landed approximately 37 barrels of cement had returned to surface. When the plug landed, pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1415 psi for 5 minutes.
1501077276 2017-07-26 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 571 rigging down and moving to a pad in Harrison County. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. Chesapeake has drained all water from the containment areas in preparation for moving the rig. Water lines have already been run to the pad to be used during the hydraulic fracturing operations. The pad drain valves are in the closed position and do not appear to be leaking. Chesapeake estimates the rig move will take approximately two days.

While I was on site I witnessed Halliburton pump the frac job for stage 5 of 96 for the 6H well. This well has had 3 stages of 46 completed. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
