0517235012 2017-08-10 Completion Testing No Violations On the date of my inspection I found a Halliburton frac crew rigged up on all 4 wells on the pad. While I was on site I witnessed Halliburton pump the frac job for stage 5 of 96 for the 6H well. This well has had 1 stage of 53 completed, and is currently screened out. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0629649082 2017-08-21 Completion Testing No Violations On the date of my inspection I found a work over rig from Stone Well Service rigged up on permit 34-081-2-0675 attempting to re-establish the ability to pump down hole by drilling out 2 stage plugs. Halliburton is also on site with a frac crew waiting for the work over rig to perform the maintenance on the wells so they can continue with frac operations. All of the wells on the pad require some maintenance before frac operations can resume. This well has been hydraulically fractured through stage 1 of 53 stages. Chesapeake anticipates the frac operations to be able to resume by 8/24 or 8/25.

1261636808 2017-06-13 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 71 is currently drilling the production string at approximately 10458 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. Fluid is leaking from beneath the containment near the half round. I tested the water using Quantabs and found it to have a concentration of greater than 6412 parts per million for chlorides. I notified the Chesapeake company man of my findings and he indicate that he would call in water trucks immediately to drain the water. He also indicated that he would have a containment crew come out to find and repair the leak. All hazardous materials are within containment areas.
1491828724 2017-04-10 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 24 inch diameter hole to a depth of 120 feet and ran 120 feet of 20 inch diameter 94 pound per foot conductor casing. Express 5 barrel of water ahead to verify circulation then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.5 pound per sack flow seal. I witnessed good cement returns to surface.

While I was on site I witnessed Halliburton pump the frac job for stage 46 of 52 on permit 3408120675 and stage 40 of 46 on permit 3408120671. They are currently on stage 45 of 53 on this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
