1136560986 2017-04-11 Casing String Witnessed No Violations I am on site today to witness the cementing of 20 inch conductor casing. Express Energy Services drilled a 24 inch diameter hole to a depth of 112 feet and ran 112 feet of 20 inch diameter 94 pound per foot conductor casing. Express 4 barrel of water ahead to verify circulation then pumped 230 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.5 pound per sack flow seal. Cement did not return to surface. Fluid level in the wellbore rose to approximately 43 feet below ground level and remained static. Express will tag cement in the morning and if less than 5 feet of water remains on top of hard cement, they will ready mix the remainder of the annulus and cellar floor.

1505825526 2017-09-19 Completion Testing No Violations On the date of my inspections I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been stimulated and are equipped with a christmas tree valve. They are not currently plumbed into the production equipment. Advanced Oilfield Services is on site currently installing the production equipment that includes three 400 barrel tanks within a metal dike, 6 gas production units, 6 glychol dehydration units, and 6 sand traps (2 spherical, 4 vertical). The pad is surrounded by an earthen dike covered with healthy vegetation with the exception of where the pipeline has recently tied into the pad. Pressure gauges on the surface and intermediate casing in pounds per square inch read at 0 and 600 respectively. Other than the equipment associated with the construction operations, 2 frac tanks and the above ground storage tank are still on location from frac operations.
1528133461 2018-06-04 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. The wells are equipped with christmas tree style wellheads. Each well has the 9.625 inch diameter intermediate casing annulus plumbed into a collection line. Surface annular pressure is 0 psi, intermediate pressure is 419 psi, tubing is 902 psi, flowline is 861 psi, and production is 1247 psi. Production equipment includes three GPUs, three sand seperators, and three 400 barrel steel produced water tanks. Tanks are in steel containment. Site is stable and vegetated. Site meets preliminary restoration requirements of ORC 1509.072A.
1555505023 2019-04-17 Preliminary Restoration No Violations On the day of my inspection I found pad to contain three producing wells. Production equipment includes three GPUs, three sand seperators, and three 400 barrel steel tanks in their own containment. Wells are producing on natural reservoir pressure and operated by a motor valve. 13.375 x 9.625 annular pressure is zero and 9.625 x 5.5 annular pressure is 40 psi. The 9.625 x 5.5 annular is plumbed into a flow line with a motor valve. Pad is stable and vegetated and meets requirements of 1509.072 (A) for preliminary restoration.
1611136028 2017-04-12 Casing String Witnessed No Violations I am on site today to witness the top out job for the conductor annulus. Express filled the remaining 43 feet of annulus with ready mix and left approximately 18 inches of ready mix in the cellar to provide a barrier between the cellar floor and soil below.
1871462049 2017-05-09 Casing String Witnessed No Violations I am on site to witness the cementing of 13.375 inch surface casing. Nomac 80 drilled a 17.5 inch diameter hole to a depth of 594 feet and set 583 feet of 13.375 inch 54.5 pound per foot casing. Schlumberger established circulation with fresh water, pumped 530 sacks of class A cement, 1.19 yield and 15.6 pound per gallon surface set cement. Approximately 28 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 814 psi for 5 minutes.
2091904983 2017-05-16 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 80 is currently drilling the production string at approximately 9280 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment area around the chemical storage and chemical mixing station has some tears and punctures. The Chesapeake company man was notified and indicated that he would have the repair crew come out this afternoon to fix the issues. All hazardous materials are within containment areas.
2110735731 2017-07-18 Completion Testing No Violations On the date of my inspection I found Halliburton hydraulically fracturing all three wells on this pad. They are currently on stage 25 for this permit number. The pad is surrounded by healthy vegetation and the drain valves are in the closed position and do not appear to be leaking. During my inspection I found that the light generators on site are not within containment. I also found a tote of wireline grease and diesel exhaust fluid outside of containment, as well as a tote of diesel exhaust fluid with no label on it. I spoke with the Chesapeake company man about the lack of containment and he indicated that they would not be changing their operations. He did indicate that a label would be added to the exhaust additive tote immediately.
