0943478770 2018-06-04 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. The wells are equipped with christmas tree style wellheads. Each well has the 9.625 inch diameter intermediate casing annulus plumbed into a collection line. Surface annular pressure is 5 psi, intermediate pressure is 260 psi, tubing is 874 psi, flowline is 848 psi, and production is 1120 psi. Production equipment includes three GPUs, three sand seperators, and three 400 barrel steel produced water tanks. Tanks are in steel containment. Site is stable and vegetated. Site meets preliminary restoration requirements of ORC 1509.072A.

1122054437 2017-04-14 Casing String Witnessed No Violations Upon arrival on location I found the crew ready to cement the conductor casing. 120 feet of 20-inch casing was run and cemented back to surface. The cementing was done by Express Energy. 43.34 bbl. of class A with 2% calcium and 1 /2 pound of flake per sack was pumped and displaced with 39.7 bbl. of freshwater. The cement returns came within 17 feet of ground level. The valve was shut, will let it set up and then grout 17 feet to surface.
1282111690 2017-07-18 Completion Testing No Violations On the date of my inspection I found Halliburton hydraulically fracturing all three wells on this pad. They are currently on stage 25 for this permit number. The pad is surrounded by healthy vegetation and the drain valves are in the closed position and do not appear to be leaking. During my inspection I found that the light generators on site are not within containment. I also found a tote of wireline grease and diesel exhaust fluid outside of containment, as well as a tote of diesel exhaust fluid with no label on it. I spoke with the Chesapeake company man about the lack of containment and he indicated that they would not be changing their operations. He did indicate that a label would be added to the exhaust additive tote immediately.

1384556225 2017-09-19 Completion Testing No Violations On the date of my inspections I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been stimulated and are equipped with a christmas tree valve. They are not currently plumbed into the production equipment. Advanced Oilfield Services is on site currently installing the production equipment that includes three 400 barrel tanks within a metal dike, 6 gas production units, 6 glychol dehydration units, and 6 sand traps (2 spherical, 4 vertical). The pad is surrounded by an earthen dike covered with healthy vegetation with the exception of where the pipeline has recently tied into the pad. Pressure gauges on the surface and intermediate casing in pounds per square inch read at 0 and 400 respectively. Other than the equipment associated with the construction operations, 2 frac tanks and the above ground storage tank are still on location from frac operations.

1495544739 2017-05-23 Casing String Witnessed No Violations On the day of my inspection I witnessed the 13.625 inch diameter surface casing cement job. Nomac 80 drilled a 17.5 inch diameter open hole to 583 feet and Wyoming casing set 573 feet of 13.625 inch diameter, 54 pounds per foot casing. Schlumberger was onsite to cement the casing back to surface with 526 sacks/112.4 barrels of class A surfaceset cement. Shoe track was 44 feet long and open hole excess of 50%. Mix water tested a pH of 7, 59 degrees F, and 31 ppm. 22 barrels of cement returned to surface and floats held with 0.5 barrel back.
1496151467 2017-05-30 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 80 is currently tripping out of the hole to repair a motor on their bottom hole assembly. They drilled to a depth of approximately 13440 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment areas around a portable fuel tank, the catwalk, and bottom of the doghouse have multiple tears and punctures. The Chesapeake company man was notified and indicated that he would have the repair crew come out this afternoon to fix the issues. All hazardous materials are within containment areas.
1601498921 2017-06-13 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 80 is currently drilling the production string at approximately 17400 feet and expect to reach their total depth on 6/14. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. I found tears in the containment around the catwalk, chemical storage, and mixing house. I notified the Chesapeake company man of my findings and he indicate that the containment crew will come out this evening to fix the issues. All hazardous materials are within containment areas.
1603637973 2017-06-07 Drill / Deepen / Reopen No Violations Jackie with Chesapeake called to notify me that they could not successfully sidetrack the well after failing to fish out lost tools. Chesapeake intends to set a whipstop in order to assist in sidetracking the well. I advised them to get the tool as close to the fish as possible within the Utica if possible. Once a plan is finalized they will notify me via email, which will be stored with the files for this permit number.
2000462676 2019-04-17 Preliminary Restoration No Violations On the day of my inspection I found pad to contain three producing wells. Production equipment includes three GPUs, three sand seperators, and three 400 barrel steel tanks in their own containment. Wells are producing on natural reservoir pressure and operated by a motor valve. 13.375 x 9.625 annular pressure is zero and 9.625 x 5.5 annular pressure is 400 psi. The 9.625 x 5.5 annular is plumbed into a flow line with a motor valve. Pad is stable and vegetated and meets requirements of 1509.072 (A) for preliminary restoration.
