0154114839 2017-08-17 Completion Testing No Violations On the date of my inspection I found Deep Well Services drilling out the plugs set by the frac crew on permit 34-081-2-0692. They are currently at 16000 feet and expect operations on this well to take until 8/21 or 8/22. After they complete drill out operations on this well they will move to this well. All chemicals are within containment. The pad drains are in the closed position and do not appear to be leaking.
0778167562 2017-05-22 Casing String Witnessed No Violations Upon arrival on location I found that the rig crew has drilled a 17 1/2 inch diameter hole to a depth of 1710 feet, and installed 1707 feet of 13 3/8 inch diameter casing with a 44.64 shoe track. Schlumberger will do the cementing. Circulation was established. 291 bbl. of cement slurry was pumped, with 100 bbl. of good cement returns to surface. Plug landed and floats held. Job complete.
0934818035 2017-08-10 Completion Testing No Violations On the date of my inspection I found a Keane Frac crew rigged up on both wells on the pad. While I was on site I witnessed Keane pump the frac job for stage 74 of 82 for this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. Ascent said they should finish up the operations on the pad in 2 more days.

1291828743 2017-06-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 84 is currently running their production casing at 19783 feet. This is the last well on the pad. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. All containment appears to be in good condition and is visibly free of tears and punctures. All hazardous materials are within containment areas.
1330467706 2017-07-20 Completion Testing No Violations On the date of my inspection I found Keane Frac rigged up on both wells on this pad. 1368308549 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 2 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 900 psi intermediate, 2200 psi tubing, and 2200 psi production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
1487263490 2017-02-11 Drill / Deepen / Reopen No Violations I arrived on location at 8:50 AM to witness the cementing of the conductor casing. The cementing was set to began at 10:00 AM but due to the hard drilling the job was backed up to 1:00 PM. I was not notified of the time change of the operation. Percision Rathole was the contractor drilling the conductor hole. Percision Rathole used a 36 auger bit to drill the hole to a total depth of 100 . Percision Rathole ran 100 of 30 120 lb/ft .312 wall thickness welded casing. There were various slots cut around the bottom of the casing at various positions and heights 3-4-5-6. Allied OFS,LLC arrived on location at 1:07PM. Ohio Water Haulers was on location with the fresh water. We had a safety meeting with Percision Rathole, Allied OFS, and Ohio Water Haulers and Stan Cherry from Ascent Resources and myself. Allied hooked up there pump lines. They 5 barrels of freshwater ahead and started there cement, they pumped 85 barrels of class A cement 15.6 lb/gal 1.19 yld. The cement was displaced with66 barrels of fresh water leaving a 20cement shoe. There was 10 barrels of cement returned to the cellar floor.
1495455574 2017-05-21 Casing String Witnessed No Violations Upon arrival on location I found the rig crew drilling ahead at a depth of 350 feet They drilled to a depth of 360 feet and a hydrostatic test was performed for 30 minutes, with no fluid loss. Test completed.
1499783834 2017-07-11 Completion Testing No Violations On the date of my inspection I found Keane Frac rigged up to begin hydraulic fracturing on both of the wells on this pad. I spoke with the Ascent company man who indicated that the operations should begin this afternoon and that they were waiting on their water supply. He indicated that they should be on this location for approximately 3 weeks. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. The company man indicated that any water that accumulates within the containment is removed using a vac truck then transferred into the water holding tank on site and used for the fracturing process. The 13.375 inch casing gauge showed zero pounds per square inch while the 9.625 inch casing gauge showed 350 pounds per square inch.

They have pumped 3 stages prior to my arrival. The cement top was tagged at 180 feet. 160 feet of 1 inch diameter tubing was run. We started seeing cement back at 47 bbl. A sample was taken and weighted. The final cement weight was 15.8 lb.
