0189695645 | 2017-10-02 | Completion Testing | No Violations | On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 11 of 56 for this well and stage 25 of 70 for permit 34-081-2-0700. Halliburton has stimulated stage 11 of 56 states on this well. During the frac, Halliburton pumped approximately 347000 pounds of sand using 6605 barrels of water. They were able to achieve pumping rates of 80 barrels per minute during the job. During the final flush of the stage, the perfs screened out and they were unable to complete the stage. Ascent had Halliburton flow back 600 barrels from the well in an attempt to re-establish flow down hole. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. |

0565650139 | 2017-03-23 | Casing String Witnessed | No Violations | The purpose of my inspection was to witness the second hydrostatic pressure test on the 24 conductor/mine string. The initial test failed due to what was believed to be a bad cement job. Prior to arriving on location I spoke via telephone with Company Representative Eugene Lanham. Mr. Lanham stated that O-Tex Pumping LLC arrived the previous night and cemented the borehole with 350 sks of Class H cement mixed at 15.6 ppg. The cement was pumped from surface through one joint of drill pipe. I witnessed Falcon 26 pick up the 17.5 bit and drilling assembly and tag the top of cement at 63 KB. The cement was drilled out of the casing utilizing fresh water. This section of well was drilled to a new total measured depth of 158.5 KB (previous TD was 154 KB) to confirm a geologic formation was not the cause of the previous test failure. I was present at the flowline and witnessed formation returns prior to giving approval to perform the hydrostatic pressure test. After reaching TD, the hole was circulated clean and loaded with freshwater. I observed the static water level at surface for 30 minutes. The water level did not fall. I communicated to Mr. Lanham that the test had passed and operations could proceed. |

Production equipment includes two 500 barrel steel tanks within their own steel containment, five line heaters, and five GPUs. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A. |

Falcon 26 drilled the 24 casing shoe out with a 17.5 to a total depth of 155 KB. At 11:42 a 120 bbl vacuum water truck began to load the open hole and casing from surface by gravity. The truck ran out of water and moved over to reload with water. Calculated total volume of the open hole and casing was 73 bbl. Company Representative Eugene Lanham lowered a plastic bottle on a string to tag the water level. The water level was tagged at 72 measured from the top of the casing converter. The truck loaded an additional volume of water down the casing. Mr. Lanham tagged the water level again at 72 at 13:45. After waiting 15 minutes, the water level was tagged again and measured at 78 feet. The hydrostatic test failed. A review of the drillers log indicated the conductor rig encountered shale and coal formations from 60-90. After discussion, the determination was made to attempt a remedial cement job by balance method followed by a second hydrostatic pressure test. I will return to location to witness the second hydrostatic pressure test. |

Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A 10 foot mine void was found from 70 to 80 feet, a basket was ran at 55 feet. Allied pumped 94 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal to set the shoe, leaving 10 feet of cement in the conductor. Gravel, bariseal, and calcium chloride were added down the backside of the 24 inch conductor to set the basket. Allied pumped 30 barrels of cement down the backside until cement remained approximately 8 feet below the cellar floor. Ascent will have the cellar topped out with ready mix. |

The pad drain valves are in the closed position however the drain on the east side of the pad near the lease road entrance is leak a small amount of water. The pad outlet pipe has some algae growth inside the pipe. The company man called the company that drains the pads (who recently drained the pad on 7/26/17) and they will be sending someone out today to determine why the drain is leaking. The company man indicated that if the leak cannot be fixed that he would have the drain valve plugged until a permanent fix can be made. |
