The pad is surrounded by a fenced earthen dike covered by healthy vegetation. There are 4 recently drilled wells on the pad surrounded by metal fences. The wells have not been hydraulically fractured yet. Production equipment on the pad includes, 4 sand separators, 4 gas production units, and two 500 barrel tanks within a metal dike. The production equipment is surrounded by concrete barriers. There is a garbage roll off container being stored on the pad near the lease road entrance. |

Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 110 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 340 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 80 barrels of water, leaving 10 feet of cement in the casing. A cement basket was ran at approximately 50 feet in case the predicted void at 60 feet opened up during the cement job. Good cement returned to surface and remained static. Cement was brought to approximately 7 feet below ground surface, leaving approximately 18 inches of good cement as a floor in the cellar. |

Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1765 feet and set 1754 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of bentonite gel spacer followed by 5 barrels of water, then pumped 1840 sacks of class A cement (1.19 yield and 15.6 pound per gallon, 1 percent calcium chloride, 0.25 pound cello flake per sack). The wiper plug was released and O-Tex displaced with 260 barrels of fresh water. By the time the plug landed approximately 100 barrels of cement had returned to surface. When the plug landed, pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1310 psi for 5 minutes. |

I was not notified of the rig move by Ascent. I spoke with the company man who thought they had notified me, and he indicated that the rig should be ready to begin drilling on 8/19/17. I informed him that they should have given me a 24 hour notice before moving equipment on to the pad. They are currently rigging up the back yard and staging the air compressors. |
1504103084 | 2017-08-30 | Drill / Deepen / Reopen | No Violations | Today when I arrived onsite to perform a well site inspection I found that the pad had been evacuated to the lease road entrance due to a hydrogen sulfide detection alarm. The gas was detected while drilling the curve at approximately 9575 feet (total vertical depth). The Utica Shale was intercepted at 9446 feet (total vertical depth). I was not notified of the incident while the well permit clearly states that the oil and gas inspector must be notified immediately when hydrogen sulfide is detected in an urban area. I spoke with the Ascent company man Matt about the mistake. After the pad was evacuated, personnel from Compliance Services entered the site with a self-contained breathing apparatus to scan all areas for gas. His detector found a concentration of 35 parts per million in the area near the shakers. The Ascent company man and 3 others entered the site to attempt to re-open the well. When they opened the valve, concentrations between 11 parts per million and 26 parts per million were detected. Ascent plans to run all gas through the flare stack to burn off the gas. They will have multiple sensors in place near the stack to detect any rogue gas emissions. After diverting the gas to the flare, they circulated a full bottoms up and by the time they had fully circulated they were not reading any more gas. |
