The pad is surrounded by a fenced earthen dike covered by healthy vegetation. There are 4 recently drilled wells on the pad surrounded by metal fences. The wells have not been hydraulically fractured yet. Production equipment on the pad includes, 4 sand separators, 4 gas production units, and two 500 barrel tanks within a metal dike. The production equipment is surrounded by concrete barriers. There is a garbage roll off container being stored on the pad near the lease road entrance. |

The floor of the cellar has deep boot impressions that appear to go completely through the cement to the soil below. When I measured from the bottom of the print to the top of the cellar, I found that there is not a sufficient enough seal between the cellar and soil below. I notified the Ascent company man about the deficiency and he arranged for a grout truck to come out to add additional cement to the floor of the cellar. Falcon 26 is currently moving onto the location to drill the surface portion of the wells on this pad. I returned later in the day to verify that the grout had been poured into the cellar and was brought to approximately 6.5 feet from the ground surface. |

Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1765 feet and set 1760 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of bentonite gel spacer followed by 5 barrels of water, then pumped 1840 sacks of class A cement (1.19 yield and 15.6 pound per gallon, 1 percent calcium chloride, 0.25 pound cello flake per sack). The wiper plug was released and O-Tex displaced with 262 barrels of fresh water. By the time the plug landed approximately 100 barrels of cement had returned to surface. When the plug landed, pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1200 psi for 5 minutes. |

During my inspection I found an area of the pad near the returns container that had been sprayed with cuttings. Also, the containment near the air compressors had a thick film with a sheen across it. The company man indicated that they would check on the chlorides before they released any water from the pad. He also indicated that he would clean up the overspray and drain the containment near the air compressors. Currently the containment crew is on site patching any visible holes. All of the chemicals were within containment. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking. |

Since my last inspection on 9/14/17, the area near the water load out and 500 barrel returns container has had new gravel placed on the ground. The area that had previously had elevated chlorides and a continuous stream of running water from under the containment is now dry. The containment does not have any visible tears or punctures. They did not encounter any hydrogen sulfide when drilling into the Utica as they did on the previous well on this pad. All of the chemicals were within containment. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking. |

Pad is within 10000 feet of jefferson airpark and ODOT has permits for erections that close to airport. A permanent solar powered light was installed on crane. Outslope of pad is vegetated and stable. Lease road is stable. Site meets requirements of ORC 1509.072A for preliminary restoration. |
