0033809213 2018-02-23 Preliminary Restoration No Violations On the day of my inspection I found this pad containg four labeled wells. Wells have surface casing set in them and are awaiting a larger rig to finish. The pad is fenced, gated, and locked. All disturbed area is heavily mulched and doing well with all the rain the area has received over the past 2 weeks. Leas road also in good shape. Pad drains are shut. Site meets requirements of ORC 1509.072A for preliminary restoration.
0036081639 2018-10-22 Completion Testing No Violations On the day of my inspection I found Deep Well snub #11 onsite. I identified myself to company man James Edwards. This well has had frac plugs drilled out and is awaiting flowback. Surface annular pressure is 450 and intermediate is 570. Production casing pressure was 3100 after drill out. James anticipates moving off of this pad late in the week.
0238550390 2018-01-09 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1467 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1460 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot and a 3 foot stickup. Casing was centralized with 11 centralizers. Shoe track is 39 feet long. Halliburton was onsite with 1466 sacks of class H Neocem cement. Cement weighed with mudscale at 15.8 ppg and yield of 1.241. 50% excess ran in open hole. Cement displaced with 216 barrels of fresh water. 10 barrels of water pumped ahead of cement. 60 barrels of cement returned to surface, and floats held with 1.25 barrels back.

This well has not had a stage done on it yet. They are waiting until to rig is off of the neighboring gordon pad. Surface annular pressure is 0 and intermediate annular pressure is 600.

Frac crew has moved off of location and they are cleaning up the containment in preperation for drill out. 2 kill plugs set in well at 9000 and 9130. Frack stacks are on wells, an aboveground storage tank, and two frac tanks are onsite. Universal will be coming to this location after finishing the Elite pad. 1010045012 2018-04-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 802 drilling an 8.5 inch diameter hoe at the top of their curve. They are just starting to kick off at 8970 feet. Company man Heath plans to land the cruve around 9900 feet. He said that 9.625 inch diameter surface job had cement to surface. There is no flare burning during my inspection. Location is clean.

Well has not been stimulated.Well has 69 stages. Stage 1 perforations will be at 21917 and top perforations at 21809. Surface annular pressure is 0 and intermediate annular pressure is 600.

1343678970 2018-09-18 Completion Testing No Violations On the day of my inspection I witnessed Universal frac crew on location. This well is on stage 33/69. Stage 33 frac plug is at 16333, bottom perforation at 16322, and top perforation at 16181. Intermediate annualr pressure is 680 psi. Surface gauge not viewed as there was pressure on the lines for frac.
1543690204 2018-09-25 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. Identified myself to company men David and Gonzo. This well is on stage 50/69. Stage 50 was just stimulated. Plug set at 13341, bottom perforation at 13330, and top perforation at 13189. Breakdown pressure was 9379 at 16.9 barrels per minute. 1051 gallons of 7.5 % HCl pumped. ISIP of 6504 and 5 minute ISIP 5560. Maximum treating pressure was 11200 and average was 10021. Maximum rate was 85.2 barrels per minute and average rate was 71.9. 5126 barrels of total slurry pumped. 51153 lbs of 100 mesh sand and 180800 lbs of 40/70 mesh for a total of 231953 lbs of sand. Surface annular pressure is 0 and intermediate is 600.
1553561007 2018-08-07 Completion Testing No Violations On the day of my inspection I witnessed Universal frac crew moving onto location. ID to Company Man Bub Ruckert. Mr. Ruckert believes they will likely start stimulation operations on Saturday. There is an above ground storage tank onsite. Not all equipment is onsite this date. Surface annular pressure is 0 and intermediate annular pressure is 900. Company man said he would have the pressure bleed off when they get all rigged up.
1843862133 2018-09-12 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. Identified myself to company man Gonzo. Stage 25 has been perforated. There are 69 total stages. Plug was set at 17741, bottom perforations at 17729, and top perforations at 17589. Well was perforated with 5 guns with 6 shots on 35 foot spacing. Wireline is coming out of the wellbore during my inspection. Surface annular pressure is 0 and intermediate annular pressure is 750.
1851113202 2018-08-14 Completion Testing No Violations On the day of my inspection I witnessed Universal frac crew on location. ID to company men Brady and Wendall. They are beginning stage 1 frac of the 6H well during my inspection. This well has not been stimulated yet. Surface annular pressure is 0 and intermediate annular pressure is 470.
1880528989 2017-12-14 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex Pumping LLC pumped 395 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack cello-flake. O-Tex then displaced with 71 barrels of water, leaving 15 feet of cement in the casing. Cement did not return to surface and remained approximately 10 feet below the base of the cellar in the annulus. Ascent will top out with grout after the cement sets up.
