0365140470 2009-12-18 Production Wells No Violations Inspect the wellsite at Mr. Mark Bordashs request. The inspection revealed that the well is being utilized by Cryrogenic Construction for gas for the shop furnace. Mr. Bordash wants to transfer the well to Cryrogenic Construction but he doesnt know if there is a viable owner for the well. He has been in contact with Central Office in Columbus about transferring the well to Cryrogenic Construction. I obtained GPS co-ordinates for the well during the inspection.I advised Mr. Bordash that I would give the GPS co-ordinates to Dis. Supervisor Cheslock and he will forward the information to Columbus. I also took photos of the wellhead and the proximity of the well to County Rd. 74. There is no tank battery associated with this well.
0809184062 2014-06-23 Plug / Plug Back No Violations Onsite with Inspector Pat Shreve and Brandon Trigg. Bishop well service was on location to plug well. Petroset is the cement contractor for this plug job. 140 sacks of class A 2% gel cement was used to plug well. Cement level dropped to ~20 below surface and will be topped off tomorrow.

0936524133 2014-06-20 Plug / Plug Back No Violations Purpose of this inspection was to witness Appalachian Well Survey perforate the ~3 casing. Bishop well service on hand with service rig. The casing was perforated at each of the following depths; 485-450-400-350-300-250-200-150-100-50.
1023353685 2013-07-24 Production Wells No Violations I met with Fred Romeo on old well that will be plugged by the owner. There is 6.625casing,4.875and 4 with 2tubing.
1368114795 2013-05-09 Production Wells No Violations This well is used by Cryogenic Inc. for their shops on location. There are no tank batteries for this well. The well is painted yellow but is located very closely to Co. Rd. 74 and could pose as a safety issue if a car were to run off the road and run into the wellhead. It took some time to find the location of this well do to incorrect GPS coordinates. I updated the coordinates for the well so that the RBDMS file can be updated.
1373563187 2013-07-10 Production Wells No Violations I was onsite to check the damage done to this wellhead when truck hit it. Currently the wellhead is bent and has some duct tape around it but shows no signs of leaking gas. I was contacted from an operator about possibly plugging this well. There are no records of this well on file so research regarding offset wells must be needed to be done.
1374108372 2013-07-14 Administrative Inspection No Violations I was contacted by Doug Gonzalez regarding the application for plug and abandon this well. The owner is going to get the paperwork together and submit it to Columbus so that the well can be plugged and abandoned.
1375977722 2013-08-07 Production Wells No Violations I was onsite to witness the gamma and collar locator logging of this well so that it can be plugged and abandoned. From the start issues were encountered. The logging tools were only able to reach 6.5 feet below surface. A tape measure was run in the tubing to try and see how far down it would go but again 6.5 feet was the max. Looking in the well with a flashlight showed that the tubing likely looks bent or some roads separated in the hole. They are going to reevaluate their options and their plans for plugging the well before any more work is done.
1403114101 2014-06-18 Plug / Plug Back No Violations Bishop well service is plugging well with Doug Gonzalez as the consultant. They have dug down to approximately 5 deep by 8wide and cut the wellhead and the 6 and 3casing off. There is wooden rods and they are broke off. Bishop is using a mouse trap to catch the wooden rods. The 2tubing has been pulled from 510and the rods are still stuck in hole. The 3and the 6are cemented in place at the top.
1403699572 2014-06-19 Plug / Plug Back No Violations Purpose of this inspection was to see if Bishop Well Services had been able to remove rods from well casing. They were able to pull the wooden rods up from 510. Operator was not able to get anything below 510.
1403801825 2014-06-23 Plug / Plug Back No Violations On the date of inspection I accompanied ODNR Inspectors Dylan Pendleton and Pat Shreve to witness the plugging of the well. Refer to Pat Shreves and Dylan Pendletons reports for further details.
1405084530 2014-07-10 Final Restoration No Violations On the day of my inspection, I witnessed that the site passed final restoration according to ORC 1509.072(B). The wellsite was situated in a concrete parking lot therefore, vegetative cover will not be established. The well owner has submitted to the Division a Landowner Waiver to waive this requirement for the wellsite and lease road.
1409274727 2013-07-24 Production Wells No Violations I was on location with ODNR inspector Pat Shreve to try and determine what is in the hole and what will need done to properly plug this well when the permit to plug and abandon is received. We determined that there is likely casing on hangers with packing in the hole, likely being 2 and 3 inside. The parking lot around the well will need dug out to try and determine what is below the wellhead. It was also agreed upon that a full cement plug would need to be cemented in this well to properly plug the well. Information from offset wells was also collected at this time to try and determine what could possibly in the well when it comes time to plug.
1427736020 2014-06-20 Plug / Plug Back No Violations Bishop ran in the day of 6-19-14 and retreived the wooded rods to depth of 485. The Appalachain wireline service came in to pererate the 3casing with 4 perfs per foot. They ran 10 guns in at every 50. They perferated the casing and will cement back to surface on 6-23-14.
1806441619 2013-08-05 Administrative Inspection No Violations Doug Gonzalez contacted me informing that they are going to be moving on location this Wednesday to log and blow down this well before they move onto the actual plugging operations. They expect to find the packer on the 2 casing and possibly find the 3 casing packer and hanger.
